Main St. Grapevine, Texas 76051 Re: Baylor Regional Medical Center at Grapevine 5t' and 6`s Floor South Tower Core and Shell; and Central Utility Plant and Engineering Offices. Dear Mr. Strombaugh, Please find attached the necessary documents for the above referenced projects per the City of Grapevine's Conditional Use Application / Site Plan Application requirements. Please note that the filing fee in the amount of $525.00 covers both the projects. 5u' and 6`" Floor South Tower Core and Shell: The proposed construction at BRMC Grapevine includes providing the core and shell for 2 (future patient room) floors to the South Tower, as well as providing a mechanical penthouse and elevator machine room at the roof level. The total added square feet is approximately 32,000 SF. The 2 story addition to the tower will change the maximum building height from 66'-0" to 90'-0" above ground level. A determination has already been made by the FAA (Federal Aviation Administration) that the project does not exceed obstruction standards and is not a hazard to air navigation. The current square foot addition to the campus also will not affect the minimum Off -Street Parking requirements as determined by the City of Grapevine. The following documents have been included as part of this package: 1. Form "A" application. 2. Form `B" application. 3. Filing fee of $525.00. 4. Address of property on 8 1/2 x 11 sheet. 5.
