B NUMBER: 5.0 PREPARED BY- Risk Management REVISED DATE: 02/01/2014 PAGE: 1 of 2 PURPOSE 5.1 The purpose of the On the Job Injuries policy is to establish procedures for immediate medical care for injured workers to ensure the most expedient recovery and return to work status. POLICY 5.2 The immediate supervisors of injured workers are to immediately transport the injured worker to minor emergency facilities if the injury is of a minor nature. The worker may be taken to: Grapevine Medical and Surgical Clinic 1501 Northwest Highway Grapevine, Texas Or CareNow 2355 Grapevine Mills Circle East or 5301 William D Tate Ave Grapevine, Texas 5.3 Should a worker receive an injury of a major nature, the immediate supervisor will immediately transport the injured worker to the emergency room at Grapevine Baylor Medical Center or call for an ambulance from the Fire Department through the Communications Center at the City of Grapevine. 5.4 The supervisor will contact the Department Head and Risk Management of the injury. 5.5 The supervisor must complete the Injury Report form (see Attachment B 5.0—On The Job Injury Report) to the Risk Management Division. The Injury Report form is also on the Grapevine Employee Intranet. 5.6 Should the injury result in lost time by the worker, the worker will furnish a physician's statement outlining the nature of the injury and days to be absent from work. 5.7 The injured worker will report weekly to the supervisor on the status of his injury while CITY OF GRAPEVINE ADMINISTRATIVE POLICY SUBJECT- ON THE JOB INJURIES SECTION. B NUMBER: 5.0 PREPARED BY- Risk Management REVISED DATE: 02/01/2014 PAGE: 2 of 2 recovering from related injuries. 5.8 The City of Grapevine may request release of injured worker for light duty and contact the physician's office to assert what light duty injured worker is capable of based on job descriptions. 5.9 The injured worker will need to provide to his/her immediate supervisor a physician's authorization for light duty. 5.10 Upon completion of the limited duty program, the rehabilitated worker will secure release to return to full active duty and permanent work status, submitting written documentation to Risk Management and the supervisor of their department.
