CITY OF GRAPEVINE THIRD PARTY REMOTE ACCESS REQUEST
INFORMATION REQUIREMENTS DOCUMENT
In accordance with the Network Connection Policy, all requests for Third Party Remote
Access must be accompanied by this completed Information Requirements Document.
This document should be completed by the City of Grapevine person or group
requesting the Third Party Network Connection.
A. Requester Information (City of Grapevine):
o Name:
o Department:
B. Purpose of Remote Access — Include a statement about the business needs of the
proposed connection; i.e. Technical support for XYZ application.
C. Scope of needs — What City of Grapevine network resource does the Third Party
Vendor need access to; i.e. File server names.
