This letter confirms on December 2, 2020 the Historic Preservation Commission staff
approved with conditions #CA20-111 for the property addressed 403 South Main Street,
legally described as Block 18, Lot 9 & 10, City of Grapevine Texas. The following was
approved on this Certificate of Appropriateness:
1. Remove existing roofing material from building as necessary;
2. Install two layers of 2/2" insulation, mechanically attached to roof structure;
3. Install a new 50 mil Duro-Last PVC deck membrane on the roof of building;
as per the attached specifications and photos with the condition a permit is obtained
from the Building Department.
An approved Certificate of Appropriateness is not an approved building permit; a
building permit is / is not required. Contact the City of Grapevine's Building
Department at 817.410.3165 for fees and information regarding permits.

• Existing roof system is to remain in place, gravel is to remain in place
• Scuppers are to be removed and replaced, downspouts are to remain in place
• Wood blocking is included at the top of parapet walls to allow for proper metal fascia
attachment. Lighting at the top of the parapet walls will need to be removed and
reinstalled by others

The 20-year NDL with 15-year Consequential Damage No Dollar Limit Warranty
provides the best coverage in the industry:
• The Duro-Last° 15-year no dollar limit consequential damage standard warranty provides the

fail during the term of this warranty

TriVAN Five Year Duro-Last® Elite Contractor Warranty
As a Duro-Last® Elite Contractor, we understand the most critical time for a new building is the first six
months of operation. During this period it is critical that operations are not interrupted by issues such as
compromised roof systems.
Our inspections will be looking for any changes to the roof system, damage caused any weather or human
intervention, any environmental conditions that may be affecting the integrity of the roof system, and any
other systems residing on the roof that may impact operations, for example covers left off of mechanical
u n its.
This will allow you to provide your owners with the continuity of support and confidence that the roof
system will be secure and will mitigate interruption of operations during that critical window of new
building operations.

Duro-last Roofing System (-Duro-Last System") installed by a Dino -Las authorized DealerlContractor ("Ccmtractar"). subject to
the conditions and limitation contained herein.

the i" through 15' year stm8 be to repair any leak in the Dum-Last System caused by any defect in a
oornpor— of the Duro-tast yysstteem or by the workmansh ep of the Contractor, but only as the workmhanship relates to the installation
of the Dttro-Lau System itselfand mix as it relates to other work performed, if any. Duro-List's obligation includes, at Dow -List's
discretion, either the repair or replacement of part or all of the Duro-Last System and also includes the furnishing or cost of labor to

E. Dum-Last authorizes the repair and. at Dum-List's option. either Dtro-Lim's QA Tech Rep(s). or an authorial Contractor
makes the repair.
Wring the 11 though 15' year of this No -Dollar limit Warranty. Owner and Duro-Last agree that this No-DoUa limit Warranty cov-
ers incidental and consequential damages derived from leaks caused by defects warranted aptrst above. During the 16" through 20'
year of this No -Dollar Until Warranty term, Owner and Duro-Last agree that Duro-Last shall not warranty the workmanship of the
Contractor, ad that Duro-Last &ball only warranty that the Dum-Last Systern (membrane, material and accessories) will not became
defective. Furthermore. during the 16' through 20' year of this No-DoOar Limit Warranty. Owner and Duro-Last agree that Duro-Last
shall not be responable for any incidental or consequential odahtagea rrclyding but not Iunited to any loss of profits, loss of use of the
building or any damage to the building or its contents. Dw o-Last meta[ receive notice in the manner set forth above, prior to the first
day of the I& year, of any claim for incidental or consequential damages, otherwise such claim is waived.

The Owner is not entitled to recover under this No-Dollm Limit Warrnty unless Owner exercises reasonable and diligent care in the
maintenance of the Duro-Last System, inclding but not Busked to inspecting and maintaining the Dum-Last System regularly and as
needed, including after storms or mural disasters, and for mmov. any debris from the Duet -Last System. rooftop. and adjacent
areas, and mainratting and keeping all drains in order and char of debris imd other obstructions.
III. LIAR ATIONSS and E.\CLLISIONS
A.This No -Dollar Limit Warranty does not apply to a Duro-Last System installed on a single-family residence.
B. Duro-Last shall not be liable for damages arising from defects in the design or construction of the building or the roof

including inadequate or insufficient drainage; nor stall Dum-last be liable for any other products aside from the
Dwo-L I System.
C. Dtro-Last is not liable for try Duro-Last System defect or failure nor for subsequent damages arising from Acts of ()oil or
causes outside Duro-LasCs eoutral including, but not limed to:
I t Damage caused by fire. lightning. hurricane, gale, Lail, omiLdo, flood. earthquake, animals, insects; or.
It Datumgt reused by accider, vandalism, intentional ace, negligence or failure to taste reasonable care, whether on the
part of the Owrer or rah(Ather or.
31 Damage cased by any a suatborized modification to the Duso-Last System including, but not limited to: damage
s caed by trstnlorized components used in installation or reams, by additional equipment or structures added to or
male a pan ofthe roof, by traffic. or by chemicals the normally fowl in nature or the like; or

F. Lhro-Lan shall have no liability under any theory of law for any claims, repairs, or (Aber damages relating to the presence of
asbestos or any vapors, Humes, molds, fu bacteria, spores. mycotoxins, or the like on or in the taro -Last System or in the
building or in the air a water serving the building.
G.This No -Dollar limit Warranty is transferable to subsequent Owners only upon the express wrinen consent of Wro-Las and
at Duro-Last's sole discretion. Dwo-Last reserves the right to require an inspection of the Duo -Last System prior to transfer
of this No -Dollar Limit W urmny. The Owner (undersigned below) mast pay a $501) warranty transfer fee and must pay for
any non •warratled repairs identified by Duro-Last during any pre -transfer inspection. A transferof this No-Dolla Lomut
Warranty shall not be effective unless all outstanding Duro-Last invoices have been satisfied_
H.This No -Dollar Lash Warninly mast be siggnneed by a Duro-Last Quality Assurance Mauer. Coverage under the terms of"
No -Anders Limit Warranty begins on the Effective Date. The Effective Date is determined by Duro-Last- Failure of the Owner
or Contractor to sign this No -Dollar Limit Warranty does" alter the Effective Date.

Coping Versus 2-Piece Compression : Next Practice
We recommend the Duro-Last° 2-Piece compression system be used
to terminate the membrane at the top of the parapet rather than
coping cap. The membrane is fastened only on the outside of the
parapet wall and meets or exceeds ES-1/ASCE-7 "Current" Standards
to 90 MPH.
Systems using coping caps require termination of the membrane
through the top and inside of the parapet as shown below and this
introduces points of failure into the roof system design. This is
exacerbated by the fact that the coping itself requires a seam every
ten feet and at corners. Each seam requires caulk to seal these seams
that shrinks and fails within just a few years. The Duro-Last° Roof
System eliminates both these issues completely.

Prefabricated Roof Membrane Layout Versus TPO
Below is an example of the engineering design for prefabrication of the Duro-Last® Roof System that will be
completed for your Roof System. The blue lines represent seams fabricated in their Carrollton, Texas
fabrication plant (one of four in the US) using automation under strict environmentally controlled conditions
(see photos below or come visit the plant!).
The dotted red fines represent prefabricated sections, welded together in the field by TriVAN's Duro-Last®
Elite Contractor team. By comparison, 100% of the blue lines would be welded in the field with TPO,
significantly increasing the margin for error in the implementation process.
Each roof system goes through a three stage inspection/quality audit (see Page 8) as an integral part of the
deployment process before a warranty is issued. Duro-Last® Elite Contractors are graded on each installation
and must maintain a high grade (92 or better) to keep their status with Duro-Last®. By comparison TPO
contractors are not held to this standard.

50 Versus 60 Mil Membranes
Where It Counts!
While it's commonly believed that thicker
membranes offer superior protection, it's
important to note where that thickness
comes from. The top layer of a membrane -
also known as the "performance" layer - is
the most vulnerable. With daily exposure
to harsh natural elements like UV rays and
unpredictable precipitation, to regular foot traffic
from mechanical and building maintenance,
membranes with inadequate performance layers
are susceptible to accelerated deterioration and
damage.
Duro-Last""s 50 mil flagship membrane is
manufactured with 28 mil of
thickness on the top performance layer,
far exceeding industry standards as well as
competitor systems with equal and even
greater mil values. In fact, ASTM 4434 industry
standards require only 16 mil of thickness above
the scrim reinforcement, allowing for extra
filler material to be added to the bottom layer
to increase overall membrane thickness. This
means that while competitor membranes will be
the same thickness, or even thicker, than Duro-
Last's overall, they don't offer protection where it
counts.
Superior Scrim
While thickness is an important factor in a roof's
long-term performance, the reinforcement scrim
layer serves as a main source of membrane
strength. Knitted in-house, Duro-Last's anti -
wicking polyester weft -insertion scrim has an
18 x 14 threads per -inch density - among the
highest in the industry. The enhanced thread
density and high tensile strength provided by
the polyester yarn offers superior puncture
resistance, while the weft -inserted tie yarn
prevents runs and doesn't allow tears to
spread. Fiberglass yarn alternatives found in
competing products have lower tensile strength
properties, allowing for easier breaks and
tears throughout the membrane. Additionally,
the antiwicking polyester thread prevents
moisture from penetrating yarn fibers, improves
welding conditions and eliminates the need
for edge sealant, enhancing the performance
and watertight integrity of a Duro-Last Roofing
System from the inside.

The Duro-Last° membrane has no filler underneath. Therefore, the membrane welds to itself. The membrane above
scrim, which is what you are buying, it thicker than any other single -ply membrane. The NDL warranty is the same for
40, 50, 60 or 80ml which all include consequential damages coverage by Duro-Last®. Also included is coverage for
water ponding & cooking grease. All coverage includes the deck, penetrations, curbs and the parapet walls!
Oh, and the membrane with cover board is Class 4 hail impact rated!!

Misters Mahan and Custable met with Historic Preservation staff to go over each of the checklist items of the PRE -CON
list. The HPO explained this was the last meeting before the Permit is issued, to go over each of the items to protect all
historic materials in place and/or removed for storage. Mr. Custable asked what were instances resulting in stopping
work and perhaps fines put in place, it was explained making his crew knowledgeable of the historic characteristics in
place and all efforts needed to not damage or destroy -if required temporary removal, to properly and securely store for
safekeeping. If any changes come about during the specified work — contact the HPO to obtain approval.
The HPO then read and explained each item on the Building Department's Code Enforcement Checklist.
It was noted Mr. Mahan is now taking bids for windows replacement, when he has decided on manufacturer (Jeld Wen)
and before final order he will apply for a Certificate of Appropriateness; upon approval Mr. Mahan, contractor and crew
will meet with Historic Preservation staff for all to go over with any questions in the required PRE -CON meeting.
Mr. Mahan will discuss windows and paint with the HPO after receiving all quotes.

Upon the Approval of a Certificate of Appropriateness WCA) NO work is to begin until:
The 1) Property Owner, 2) Contractor, 3) Historic Preservation Officer and 4) Building
Department Inspector have met with a mutual understanding of the Historic Preservation
Ordinance as applied to this property. (Ord. No. 2013-23, § 2, 6-4-13).
Sec. 12. - Penalties. any person violating any of the provisions of this ordinance shall
be deemed guilty of a misdemeanor and upon conviction thereof shall be fined in a sum
not to exceed $2,000.00 and a separate offense shall be deemed committed upon each
day during or on which a violation occurs or continues.

This documented meeting is to aid in Grapevine's historic properties construction process
to preclude any construction errors or misinterpretations and is not intended to be a
comprehensive list of all Historic Preservation Ordinance requirements
1. A pre -construction meeting shall be held with the 1) Property Owner, 2) Contractor, 3)
Historic Preservation Officer and 4) Building Department Inspector to review the Historic
Preservation Ordinance and the requirements of the Approved Certificate of
Appropriateness. A copy of the signed Historic Preservation Officer's notes from the
meeting are a part of this document and must be attached. Signatures certifying this
meeting are required below.
2. Additional Certificates of Appropriateness may be required for a Landmarked Property
for exterior work as established in the initial Certificate of Appropriateness. This includes
all exterior finishes, paint color selection, exterior lighting fixtures, doors and door
hardware, windows, porch posts and railings, etc. In most cases, additional #CA's are
required beyond the first approved #CA for the structure itself.
3. After the meeting, no changes from the approved #CA are to be made to the building
or site without submitting revised plans for approval by requesting an additional
Certificate(s) of Appropriateness. Such changes would include siding removal,
alterations to exterior walls, door and window framing, replacing entry and garage doors
and hardware, windows or changing any building detail not specified on Historic
Preservation Approved plans.

4. It is the contractor's responsibility for any existing preservation materials removed to
be stored, secured and kept safe at all times until reattached.
5. The contractor named on this document will be held responsible to notify all in his
employ, to include sub -contractors and tradesmen, of red lined items on the plans with
any approved changes.
6. Any proposed changes to Historic Preservation Commission approved plans, must
return before the Commission, at a regular meeting, for approval. Allow 42-83 days for
this process. Contact the Historic Preservation Department to determine applicability.
City of Grapevine business hours are Monday through Friday, 8 a.m. - 5 p.m.
The required pre -construction meeting will be scheduled as quickly as possible following
the property owner's call to the Historic Preservation Office, 636 South Main Street,
817-410-3197 or 817.410.3185. This meeting is required before any work, clean up,
demolition or changes are made to the property listed above.

PRE -CONSTRUCTION CHECKLIST
PROJECT ADDRESS: -
***This is not intended to be a comprehensive list of all city requirements, but is meant to be a
guide to aid in the construction process.
• Provide clear and fully legible project site address sign, readable from the curb.
• Working hours in the city are from 7AM to 7PM Monday through Saturday. No work is
allowed on Sunday.
• No work may be covered prior to receiving written approval from the inspector. All
inspection approvals should be kept on site with the approved drawing packet by the
project manager.
• Inspections requested prior to 7:30 am will usually be performed the same morning.
Inspections requested by 12:30 pm will usually be performed that afternoon. Inspection
request phone number is 817-410-3010. After hour inspections or Saturday inspections
require 48-hour advance approval and fee payment. Minimum $84.00.
• Provide a copy of the Setback Survey for city of Grapevine approval. Your surveyor may
fax this to the building department at 817-410-3012. (for additions/alterations affecting the
building footprint).
• No inspections on the foundation will be made prior to city approval of a Setback Survey.
• No changes from the city approved plans may be made to the site or building without first
submitting revised plans and receiving written approval from the historic and building
departments. This includes signs, landscaping, lighting, etc.
• Note: Projects that went through the Historic Commission approval process must go
back through Historic Commission for most exterior changes. Allow 30-45 days for this
process. Contact David Klempin to determine applicability.
• A building permit final approval will not be issued prior to written approval from historic,
and public works departments.
• All work shown on the approved plans must be complete before a final approval will be
issued. This includes landscaping, screening, grading, signage, etc.
• You are advised not to call for your final inspection on the day the occupancy is desired.
Due to the complexity of most projects, it is very common for the initial final inspection to
fail.

• Republic Waste is the exclusive waste hauler for the city and is the only one you may use.
Any other waste receptacles will be cited and ordered removed immediately.
• Sanitary facilities shall be available at the location where the work is performed as well as
hand sanitizers for the workers. Do not go to adjacent businesses for sanitation.
• All contractor and sub -contractor parking is required to be located on the project site.
• All weather access to the site shall be provided for the city inspectors and emergency
responders.
