The steel reinforcing bars should conform to ASTM A615/Gracie 40. The pier cap should be approximately 6" thick x 18" wide x 18" deep and contain a minimum of nine #3 rebars, Five bars should be installed In the bottom V3 of the cap and four bars installed in the top of the 1/3 of the cop. The top bars should be placed at right angles to the bottom barn After the piers and Gaps have cured for at least seven (7) days, mechanically raise the foundation to its original position and installs O -Sack mix 501101 concrete Nocks between the caps and grade beam. Fine leveling should be accomplished using ONLY steel plate shifts (4".x 4" minimum). The repairs were performed by Fox Foundation Repair Co,, LLC' (Fox). Six (6) exterior and two (2) interior drilled piers with pier caps were installed under the perimeter grade beam in the 1Qcations and per the specifications Indicated in mX November 26, 2012 engineer's report (copy on file). These piers were single shaft piers (12" dia, shafts), spaced on approximate six (g) foot intervals, and extended to depths of 12 feet. (NOTE: design specifications called for completion depth of 12 feet or to shale. No shale layer was reportedly encountered at the completion depth(s) of 12 feat). The piers were drilled Gild poured on /about December 21, 2012, and the affected area of the foundation was raised on Docomber 28, 2012. After the piers and caps had cured sufficiently, the affected area of the foundation was mechanically raised as close as passible to its original position, 6-sack mix concrete blocks were installed between the pier caps and grade beam. Fine leveling was performed using 4" x 4" steel plate shires. Excess fill dirk w,9s placed at the perimeter of the foundation, where needed, and sloped so as to provide positive drainage away from me structure. Results of pre- and/or post - repair plumbing tests were not available as of this date, The repairs made by Fox were judpad to have been performed correctly and have adequately supported the previously affected perimeter areas of this foundation.
