At the- reques`f of Perma -Pier Foundation Repair Company, We performed an assessment of the
3916 Meadow Drive, Grapevine, Texas 76051, on July 9, 2007. The scope of the assessment
includes, the site inspection and verification of the plan of repair and procedure to be employed
to correct the condition noted on the survey. A final assessment of the result of repair will be
performed so that the work can be certified.
The structure is a single- family one -story dwelling with a brick veneer supported by a reinforced
concrete slab on grade foundation. For purposes of this report, the structure is deemed to face
south.
A plan of repair has been prepared and recommended retaining walls be placed as shown on the
attached drawings 70709 -0 -IA in order to made the home's foundation structurally stable. The
retaining wall specifications 70709 -09 -2 are also enclosed for the project.
This report is a presentation of professional opinion and is prepared solely to determine the
structural integrity of the structure. The inspection was made by visual observation only and
information available at the time of the investigation. Except as expressly noted, compliance
with Building Codes and other specifications are specifically excluded from the site
investigation. No responsibility is assumed for the events that occur after the inspection and
submission of this report and no warranty, either expressed or implied, is made.

Dear Sir:
Our records indicate that Perma Pier Foundation Repair was issued a foundation
repair for the above mentioned address. The paperwork submitted for review
indicated that your company was the engineer for the foundation repair. We are
looking for a final engineer report for the completed work at this address. We
have tried contacting the company Perma Pier with no success. In the document
your company provided for review it stated, "A final assessment of the result of
repair will be performed so that the work can be certified." We would like to
obtain a copy of the final report so we can close out this permit. Your help in this
matter would greatly be appreciated.
Enclosed is a copy of the permit issued to Perma Pier Foundation Repair which
has expired.

To whom it may concern,
The residence located at 3916 Meadow Drive, Grapevine, Texas 76051 is a one -story slab
on grade structure that has experienced some foundation movements. we inspected the
structure on July 9, 2007 and prepared the repair plan drawing 70709 -09. Perma -Pier Foundation
Repair company has installed the piers to support the foundation as shown in the drawing.
The piers are properly placed and seated and the jacking has been properly completed per the
plans. The structure has been raised into more level position and is now supported on the single
shafted concrete press pilings and steel shims.
The complete leveling of the affected areas should not be expected. Foundations are often built
out of level. Soils in this area are conducive to movement, expansion &/or contraction. The soil
movement could possibly cause the foundation to move, even after repair. Some slopes
remained after the repairs. This foundation repair is effective for the existing floor plan layouts
only. This repair work may not support any alteration of the floor plan and re- assessment is
suggested prior to a floor plan alteration.
Plumbing leak test for all utility lines should be performed annually and any detected leak should
be repaired immediately. Drainage around the perimeter of the home & around the retaining wall
should be maintained, so that water flows away from the structure to a positive drainage flow.
I hereby certify that the conclusions contained in this report have been, to the best of my
knowledge, correctly and completely stated without bias, and are based upon any observations
and experience. No responsibility is assumed for the events that occur after the inspection and
submission of this report and we cannot guarantee your satisfaction with future foundation
performance to any portion of this structure. This certification is void if the floor plan is altered.
Limit of liability shall be the fee paid for this report.
