BAYLOR REGIONAL MEDICAL CENTER AT GRAPEVINE
1. Please add case number CU11-28 and label each sheet in
consecutive order (ex. Sheet 1 of 2) within the signature block in
lower right hand corner of each sheet. Also please provide sealed
plans of the final revision and a copy of the proposed amended plat.
2. Please label the architectural site plan as dimensional control site
plan. In addition to the architectural site plan please provide an
overall site orientation plan which shows Lancaster Drive as it
intersects with College Street along with the footprints of the
existing buildings/remaining parking which ring Lancaster Drive.
Please show the location of Lancaster Drive, Highway 114 and
West College Street within the key plan.
3. Please show the property line on the dimensional control site plan
and site orientation plans with a heavier weight line which coincides
with the property line on the proposed amended plat. Please label
the building setbacks appropriately as shown on the proposed
amended plat.
4. Please label all existing buildings on both the dimensional control site plan
and the overall site orientation plans. Please dimension the distance of
the proposed new tower expansion to the west, south and east property
lines. Please incorporate into the parking tabulation chart the existing bed
count and the number of required parking spaces, the bed count at the
completion of the proposed expansion and the number of required parking
spaces, and the bed count at capacity and final required parking space
count. Please depict on the site plan and in the parking tabulations chart
the employee, doctor and patient/visitor parking within the surface and
structure parking areas.
5. Please add a of five-foot sidewalk on both sides of the existing driveway
that extends Baylor Parkway north of Lancaster Drive and continue the
sidewalk around corner to west to meet pedestrian crossing opposite new
main entrance, and around the corner to the east to proposed crosswalk
to parking lot. Please contact the Transportation Manager -Mike
Pacelli at 817.410.3361 for questions regarding the preceding
comment.

6. Please provide a temporary entrance plan. Please contact Deputy Fire
Chief Randie Frisinger at 817.410.8100 for questions regarding the
preceding comment.
7. Please label existing signs on the dimensional control site plan and
indicate the distance from property lines.
8. The number of perimeter trees within the landscape calculations chart has
been omitted. Please provide the data and indicate the number of trees
displaced and added based upon the proposed expansion.
9. Please show the right-of-way for SH -114 and label the purpose/function of
the lines that are shown adjacent to the west property line.
10. Some existing utilities will need to be relocated. Please contact Scott
Dyer or John Robertson at (817) 410-3136 regarding utility questions.
11. Please provide a request letter that refers to the proposed expansion as a
Conditional Use Application instead of a Special Use Application.
12. The overall site plan and proposed building elevations should
incorporate the current expansion and future expansions.
13. Please provide unshaded/uncolored building elevations of the proposed
expansion and include a portion of the existing elevation to provide a point
of reference.
14. Please note on the site plan that all on-site electrical conductors
associated with new construction shall be located underground.
15. Please note of the site plan that all the requirements of the Grapevine Soil
Erosion Control Ordinance shall be met during the period of construction.
16. For the final revision please submit the overall site plan, dimensional
control site plan, landscape plan, exterior elevations, temporary entrance
plan, photometric plan and the proposed plat in that order. The other civil
plans may be omitted.
17. Please add the following note stating the purpose of the request;
Conditional Use request CU11-28 is request to amend the
previously approved site plan of CU07-15 (Ord. 07-40),
specifically to allow a four story 167,939 -square foot
expansion to the front of hospital.
18. Future expansions may be referenced within the preceding note.
19. For verification purposes please email a PDF of the final plans to
tri lett _ rap )vInetexas._ prior to producing the final copies.

20. The final revisions including any text and data should be legible on an
11 "x1 T' sheet.
The above comments must be reflected in the revised submittal,
which is due by Wednesday, September 28th at 12:00 p.m. for the
October 18, 2011 joint public hearing. Please submit the revised
documents to the Department of Development Services at 200 South Main
Street.
The revised submittal shall include:
one (1) mylar of each sheet (minimum size 22" x 34")
one (1) colored rendering of the landscape plan (absent text) on 11" x 17"
sheet or a digital version of the same
two (2) folded blackline copies of each sheet, (minimum size 22" x 34")
forty (40) reduced sorted (collated sets) copies at 11" x 17" three hole
punched, stapled and z -folded (fold the right edge to the left edge
and then fold the right edge in half to form a z -fold).
If these revisions are made and submitted at the correct time, you will be
scheduled for the next available agenda.

Plan shows hike/bike trail connecting into street at north end of property. Trail needs to be aligned to
allow for future extension beyond this property without requiring reconstruction of their newly installed
work.

Albert,
Unfortunately, I think that the requirements are a little fuzzy in this case.
The intersection is an all -way stop, so it can be argued that normal sight triangles are not necessarily
required. But its a good idea to try to get them anyway, because we never know if the type of traffic
control may change in the future. However, because of the curve on Lancaster to the east, that sight
triangle would go all the way back past the building setback line, so that would seriously impact the sign
location. I am also concerned with the mass of landscaping proposed around the signs, since that may
have a greater effect than the signs themselves.
think that a reasonable compromise would be to say that the signs and any landscaping over 3 ft above
the pavement need to be kept back at least 15 ft from both curb lines. That is loosely based on the 14.5 ft
setback distance that we use when measuring typical sight triangles. They are already fairly close to that,
but may need to move back away from the street a little farther.
If there are any concerns, I'd be happy to talk to the site engineer if necessary.

Mr. Triplett - Thanks for your help getting our application turned in last Tuesday. Based on the owner's
inquiry to us, we wondered when we might expect review comments from the City on our application?
Also of note ..... we need to let you know we may need to seek a variance in our impervious coverage and
open space numbers. Two reasons - short of openness number by between land 2% based on our
recent calculations and the total site area will decrease making that percentage grow once the TXDOT
and Baylor right of way deal is complete. Does the City allow greenspace on a roof top to be considered
as "open or landscape" area?

On Tuesday evening, October 18, 2011 at 7:30 P.M. in the City Council Chambers, 2nd
Floor, 200 South Main Street, Grapevine, Texas, the City Council and Planning and Zoning
Commission of the City of Grapevine will hold a public hearing to consider the following
items:
Z11-06, CU11-26, PD11-06 - Grapevine Terrace Apartments - submitted by SPK
Development Partners property located at 2311 West Grapevine Mills Circle and proposed
to be platted as Lot 1, Block 1, Grapevine Terrace Apartments Addition. The applicant is
requesting to rezone 11 acres from "R -MF -2" Multifamily District to "R -MF" Multifamily
District for the development of a 4 -story apartment complex. The applicant is also
requesting a conditional use permit for density, height and parking and a planned
development overlay to include but not be limited to deviation from maximum length of
buildings and distance between buildings. The property is zoned "R -MF -2" Multifamily
District and is owned by City Bank.
CU11-27 - Grapevine Ford -Lincoln - submitted by Carroll Architects for property located
at 801 East State Highway 114, and platted as Lot 2, Block A, VTG Grapevine Addition.
The applicant is requesting a conditional use permit to amend the previously approved site
plan to remove the used car sales building and allow the addition of a new car sales
showroom for Lincoln and a pole sign. The property is zoned "PCD" Planned Commerce
Development and is owned by AIGTLP-Texas LP.
CU11-28 - Baylor Medical Center - submitted by Baylor Medical Center for property
located at 1650 West College Street and platted as Lot 1A, Block A, Grapevine Medical
Center Addition. The application is requesting a conditional use permit to amend the
previously approved site plan to allow an expansion to the hospital. The property is
currently zoned "CC" Community Commercial District and "PCD" Planned Commerce
Development District and is owned by Baylor Regional Medical Center.
HUYJ1*2 - D E Box Addition - submitted by City of Grapevine for property located
generally south of East Wall Street, west of Ruth Street, south of East Worth Street and
east of Eckley Street and platted as the Lots 1-6, Block 1, Lots 1-16, Block 2, Lots 1-6,
Block 3, Lots 1-6, Block 4, Lots 1-16, Block 5, Lots 1-4, Block 6, Lots 1-8, Block 8, Lots 1,
2A, 213, 3R, Block 9, D E Box Addition. The applicant is requesting designation as a
historical landmark sub -district. Such sub -district may include buildings, land, areas, or
districts or historical, architectural, archaeological or cultural importance orvalue that merit

REQUESTED CONDITIONAL USE AND COMMENTS:
The applicant is requesting a conditional use permit to amend the previously approved site
plan of CU07-15 (Ord. 07-40) specifically to allow a four-story, 167,939 square -foot
expansion to the front of the hospital.
Baylor Regional Medical Center of Grapevine expansion plans include a new tower and
main entry both on the south side of the hospital, and a bridge connector from the west
side of the hospital to the existing parking garage. With this request the applicant
proposes to add a new four-story tower, 85' above ground level which upon completion will
add up to 109 additional patient beds, allow for enlarged cardio vascular services and
expanded radiology services. A possible fifth floor, up to 1 00'above ground level may be
added to the tower in a future phase. At the conclusion of the expansion, 1,875 parking
spaces will be available including 1,029 patient/visitor parking spaces. Required parking

for the existing uses on the subject site, including the proposed hospital expansion is
1,240 -spaces.
PRESENT ZONING AND USE:
Current zoning on the property is "PCD", Planned Commerce Development and "CC"
Community Commercial and is currently developed with a hospital, medical office tower
helistop, and central utility plant.
HISTORY OF TRACT AND SURROUNDING AREA:
The subject property was rezoned in 1982 (Z82-34) from "PD", Planned Development
District allowing for multifamily dwellings, community business, light industrial, off-street
parking, a greenbelt recreational area to "SP" Specific Use Permit District for the
construction and operation of a multiple story professional building, construction and
operation of a multiple story patient bed tower and the expansion and operation of a power
plant all incidental to the operation of a medical center complex, providing the height does
not exceed the maximum height permitted by the Dallas/Fort Worth Regional Airport
Zoning Ordinance, 756 feet or seven floors; whichever height is lesser of the permitted
heights all in accordance with a site plan. The "SP" Specific Use Permit District was
amended in 1989 with the location of a future physical plant and future helistop site and
future 4 -story expansion to patient tower (CU89-07) and a location of a helistop pad (SU89-
02). The Specific Use Permit District containing 15.288 acres was expanded to add
10.9866 for a total of 26.2746 acres (CU92-06) in 1992 to add a new physical plant and
expand the parking area. The District was amended by CU94-30 in 1994 to develop a one-
story 12,830 square foot family clinic. CU95-04 amended the site plan to expand the
development for the outpatient facility emergency room, diagnostic radiology and medical
imaging, women's service center and intensive care. SU96-11 allowed for six wireless
communications antennas and associated equipment to be placed atop the Professional
Building. CU96-32 amended the site plan to expand the Women's Center. SU96-15
allowed for three wireless communications antenna arrays of three panels each (nine total
panels) and associated equipment panels to be placed inside the elevator equipment
penthouse. SU97-05 was approved on March 18, 1997 for three wireless communications
antenna arrays of three panels each (nine total panels) to be placed on the sides of the
Professional Building with an equipment shelter to be placed on the roof. CU97-11
amended the site plan to allow a 300 square foot addition to the Dietary Center,
modifications to the patient drop-off drive and canopy, and an addition of a 115 -vehicle
parking lot. Conditional Use Permit CU98-37 (Ord. 99-05) allowed the expansion of the
existing hospital facility, the addition of a 436 vehicle parking garage, an additional dock
drive, and two (2) temporary surface parking lots (not to exceed 18 months from date of
approval). Conditional use request C000-23 amended the site plan specifically to convert
the approved 54 temporary parking spaces on the northwest side of the property to
permanent parking for the Baylor Hospital. At the November 21, 2000 meeting Council
approved conditional use request C000-55 (Ord. 00-121) specifically to amended the site
plan to allow the conversion of a previously approved four-story addition to a five -story

addition and the conversion of a previously approved one-story addition to a two-story
addition. At the June 19, 2001 meeting Council approved conditional use request CU01-26
(Ord. 01-48) specifically to amend the site plan to add a sixth floor to previously approved
five floor administration/patient facility and locate a new generator on the lower level of the
existing parking garage. Conditional use request CU05-55 amended the site plan
specifically to allow for a 1.90 acre, 145 space parking lot for the existing hospital facility.
Conditional use request CU06-09 amended the site plan specifically for an expansion in
excess of 6,000 square feet to the emergency services area of the medical complex.
Conditional use request CU06-30 amended the site plan specifically to add shade
structures to cover a designated handicap parking area adjacent to the south side of the
existing parking garage. At the July, 2007 meeting Council approved conditional use
request CU07-15 (Ord. 07-40) on the subject site which allowed for a two story addition to
the south tower and the development of a central utility plant. At the May 20, 2008 meeting
Council approved conditional use request CU08-11 (Ord. 08-26) and special use request
SU08-02 (Ord.08-27) respectively, on the subject site which allowed for a five story parking
garage and a helistop. The Site Plan Review Committee at their October 28, 2009 meeting
approved conditional request CU09-39 to allow for revised drive aisles and surface parking.
The "R-12.5", Single Family, "R-3.5", Duplex, and" PO", Professional Office areas to the
north were rezoned from "R-1", Single Family, "R-2", Two Family, "R-3", Multifamily and "C-
2", Community Business in the 1984 City Rezoning.
The "CC", Community Commercial area to the south was rezoned from "PCD", Planned
Commerce Development in 1987 (Z87-09) and from "PD", Planned Development in the
1984 City Rezoning. The Planned Commerce Development area to the south was rezoned

The subject tract is located within "Zone A" Minimal Zone of Effect as defined on the
"Aircraft Sound Exposure: Dallas/Fort Worth Regional Airport Environs" map. Few
activities will be affected by aircraft sounds in Zone A, except for sound sensitive activities
such as auditoriums, churches, schools, hospitals, and theaters. The proposed additions
are appropriate in this noise zone.
MASTER PLAN APPLICATION:
The Master Plan designates the subject property as Commercial and a small area to the

This letter is to verify that your request for a conditional use permit located at 1650 West College
Street and platted as Lot 1A, Block A, Grapevine Medical Center Addition, to amend the approved
site plan of CU07-15 (Ord. 07-40) specifically to allow a four-story, 167,939 square -foot expansion
to the front of the hospital was approved by City Council on October 18, 2011. A copy of the
approved site plan and Ordinance 2011-57 is enclosed.
On October 18, 2011, the Planning and Zoning Commission recommended the City Council
approve the request.
Any changes to a site plan approved with a conditional use or a special use (no matter how
minor or major) can only be approved by city council through the public hearing process.
No Conditional Use Permit shall be valid for a period longer than one (1) year from the date on
which the City Council grants the Conditional Use, unless within such one (1) year period: (1) a
Building Permit is obtained and the erection or alteration of a structure is started, or (2) an
Occupancy Permit is obtained and a use commenced.
The City Council may grant one additional extension not exceeding one (1) year, upon written
application, without notice or hearing. No additional extension shall be granted without complying
with the notice and hearing requirements for an initial application for a Conditional Use Permit.
