On August 15, 2006, the City of Grapevine Planning and Zoning Commission recommended that the City Council approve the original request for the permit. Enclosed herewith is a copy of the letter regarding the approval of the original permit. We inadvertently let the permit expire, but an extension of the permit is required in order to have additional time to secure financing for the project. Any changes to a site plan approved with a conditional use or a special use (no matter how minor or major) can only be approved by city council through the public hearing process. No Conditional Use Permit shall be valid for a period longer than one (1) year from the date on which the City Council grants the Conditional Use, unless within such one (1) year period: (1) a Building Permit is obtained and the erection or alteration of a structure is started, or (2) an Occupancy Permit is obtained and a use commenced. The Doctor's office will be used by Dr Phillips Kirk, Labor, M.D. and his staff at Eye Consultants of Texas for general ophthalmology and lasik procedures. In addition to the office area, there will be a State Licence Ambulatory Surgical Center with two operating rooms and associated spaces as required. The center can be used for any surgical procedure not requiring overnight stay. Dr Labor will use the center for cataract and refractive surgery. The building is designed to promote the idea of a progressive, state of the art medical facility located within a warm, safe and caring environment and has a construction cost budget of approximately $174 per square foot. The exterior of the building will be composed of brick masonry, natural stone and stucco; while the sloped roof areas will be standing seam. Interior finishes include: granite counter tops; wood, stone and carpeted floors; solid wood doors; aluminum insulated glass windows; acoustic, plaster and gypsum ceiling and other finishes typically associated with a Class "A" Building of this type. It is my opinion that this building will become a noted landmark in the area and will not harm the value, use or enjoyment of other property in the adjacent neighborhood.
