All Conditional Use and Special Use Applications are assumed to be complete
when bled and will be placed on the agenda for public hearing at the discretion of
the staff. Based on the size of the agenda, your application may be scheduled to a
later date.
All public hearings will be opened and testimony given by applicants and
interested citizenry. Public hearings may be continued to the next public hearing.
Public hearings will not be tabled.
Any changes to a site plan (no matter how minor or major) approved with a
conditional use or a special use permit can only be approved by city council
through the public hearing process.
Any application for a change in zoning or for an amendment to the zoning
ordinance shall have, from the date of submittal, a period of four months to
request and be scheduled on an agenda before the Planning and Zoning
Commission and City Council. If after said period of four months an application
has not been scheduled before the Commission and Council said application shall
be considered withdrawn, with forfeiture of all filing fees. The application, along
with the required riling fee may be resubmitted any time thereafter for
reconsideration. Delays in scheduling applications before the Planning and
Zoning Commission and City Council created by city staff shall not be considered
a part of the four month period.

GRAPEVINE MEDICAL OFFICES
Please use the address of 815 Ira E. Woods Avenue and replace
case number CU07-35 with CU09-31 within the signature block
of each sheet.
Please provide a revised master site plan (Contact Lawrence
Cates at (972) 385-2272 for the master site plan) with the
single address, new case number of CU09-31 and include a
revised project data chart for Lot 17 and an updated building
footprint for the subject site. Also please add the following note
to the master site plan and to the site plan and other sheets for
the subject property:
CU09-31 is a request to amend the previously approved site
plan of CU07-35 (Ord. 2007-66) for a planned commercial
center, specifically to revise the building elevation and
building footprint of one office building.
Please remove the old note from sheet 1 of 4 referring to the
development of two medical office buildings.
Please provide a dimension control site plan which dimensions
the distance of the proposed office building to the north and west
property lines. Also please dimension the distance to the front
property line for the proposed monument sign.
5. Please note on the site plan the following:
• All requirements of the Grapevine Soil Erosion Control
ordinance shall be met during the period of construction.
• All on-site electrical conductors associated with new
construction shall be located underground.
6. Please remove the proposed zoning line from the site data
summary chart.
7. The parking ratio for medical office uses (150/gross floor area +
5) requires five (5) parking spaces in addition to 63 shown in the
site data summary chart. Please revise the chart accordingly.
The above comments must be reflected in the revised submittal,
which is due by Thursday, August 20th. Please submit the revised
documents to the Department of Development Services at 200 South Main
Street.
The revised submittal shall include:
four (4) folded blackline copies of each sheet, (minimum size 22" x 34")
six (6) reduced sorted (collated sets) copies at 11" x 17" stapled and z -
folded (fold the right edge to the left edge and then fold the right
edge in half to form a z -fold).
If at all possible please email a copy of the revisions before the final hard copies
are made. If these revisions are made and submitted at the correct time, you will
be scheduled for the next available agenda.

Staff recommends the Site Plan Review Committee consider the proposed changes to
Conditional Use Request CU07-35 (Ord. 2007-66) approved at the November 20, 2007
meeting for a planned commercial center, specifically to allow changes to the square
footage and elevations of a medical office building, and take any other action
necessary.
At the October 17, 2006 meeting Council approved CU06-39 which was an amendment
to the planned commercial center to allow the development of two medical office
buildings totaling 6,595 square feet. At the November 20, 2007 meeting Council
approved CU07-35, an amendment to allow the development of two additional medical
office buildings totaling 9,456 square feet.
With this request the applicant proposes to revise the exterior building elevations,
enlarge the building footprint and create office space within the second floor. The
proposed building footprint has increased from 4,658 square feet to 5,485 square feet
and the total usable square footage has increased from 4,658 square feet to 6,752
square feet. The additional office space is possible because of the conversion of the
attic into office space. The proposed office will house medical and professional office
uses. Please see the attached letter and plans.

This letter is to verify that your request to amend the previously approved site plan of
Conditional Use Request CU07-35 (Ord. 07-66) for a planned commercial center, specifically
to revise the building elevations and building footprint to allow 2,852 square foot of medical
office space and 6,900 square foot of professional office space located at 815 Ira E Woods
Avenue and platted as Lot 17, Block 1, Towne Center Addition #2 was approved by the Site
Plan Review Committee on August 26, 2009. A copy of the approved site plan is enclosed.
Please be advised that due to parking restraints, the amount of space devoted to
medical office will be closely monitored.
Any changes to a site plan approved with a conditional use or a special use (no matter
how minor or major) can only be approved by city council through the public hearing
process.
No Conditional Use Permit shall be valid for a period longer than one (1) year from the date on
which the City Council grants the Conditional Use, unless within such one (1) year period: (1) a
Building Permit is obtained and the erection or alteration of a structure is started, or (2) an
Occupancy Permit is obtained and a use commenced.

The following list of changes to the originally approved Site Plan are requested so as to
allow the Medical Practice desiring to build and locate on this property the spactial
requirements they need to run their practice properly. These changes have been required
to meet their needed program due to TDLR, State of Texas handicap laws, changing the
way they allow second floor space to function without an elevator. The overwhelming
cost of an elevator requires the building to have a larger footprint to balance this cost.
The changes are as follows:
1. Allow a larger ground floor foot print by moving the building lines on the north
and west property lines from eighteen feet to fifteen feet.
2. Revisions have been made to the Site Data Summary Chart with respect to
Percentage of open space and impervious area. The original plan numbers did not
include the square footage of pavement located in the drives to the south and west
of the site. New numbers still meet and exceed the minimum requirement.
3. Revisions have been made to the Site Data Summary Chart with respect to the
Breakdown of required Parking based upon the building containing Medical and
General Office usages.
4. Revisions have been made to the Site Data Summary Chart with respect to the
Building now being a two story structure in number of floors although the second
floor is "tucked" into the "attic" space or lines of the roof.
5. The Exterior Elevations still use the same materials that match the development's
Standard materials: stone veneer, standing seam metal roof, etc. The actual
elevations still reflect the same concept as before but are slightly modified to
reflect the new foot print of the building and the addition of a third entry feature
on the East Elevation.
