All Conditional Use and Special Use Applications are assumed to be complete when filed and
will be placed on the agenda for public hearing at the discretion of the staff. Based on the size
of the agenda, your application may be scheduled to a later date.
All public hearings will be opened and testimony given by applicants and interested citizenry.
Public hearings may be continued to the next public hearing. Public hearings will not be
tabled.
Any changes to a site plan (no matter how minor or major) approved with a conditional use or
a special use permit can only be approved by city council through the public hearing process.
Any application for a change in zoning or for an amendment to the zoning ordinance shall
have, from the date of submittal, a period of four months to request and be scheduled on an
agenda before the Planning and Zoning Commission and City Council. If after said period of
four months an application has not been scheduled before the Commission and Council said
application shall be considered withdrawn, with forfeiture of all filing fees. The application,
along with the required filing fee may be resubmitted any time thereafter for reconsideration.
Delays in scheduling applications before the Planning and Zoning Commission and City
Council created by city staff shall not be considered a part of the four month period.

2. Please add the following note to the site plan:
Conditional use request, CU07-18 is a request to amend the
previously approved site plan of CU06-29 (Ord. 06-54) for a
personal care facility and long-term acute care hospital, specifically
to modify the building elevations.
3. Please include a dimension control site plan, landscape plan and
highlight the proposed building elevations changes with the final
submittal.
4. Please include in a tabular format the total square foot material
calculations for the stone/stucco facade and the metal panel fagade.

5. Please provide a letter describing the proposed amendments to the
approved site.
The above comments must be reflected in the revised submittal,
which is due by Thursday, July 26th at 12:00 p.m. for the August 21st
public hearing. Please submit the revised documents to the Department
of Development Services at 200 South Main Street.
The revised submittal shall include:
one (1) mylar of each sheet (minimum size 22" x 34")
two (2) folded blackline copies of each sheet, (minimum size 22" x 34")
forty (40) reduced sorted (collated sets) copies at 11" x 17" three hole
punched, stapled and z -folded (fold the right edge to the left edge
and then fold the right edge in half to form a z -fold).
If these revisions are made and submitted at the correct time, you will be
scheduled for the next available agenda.

Medical Addition. The addition contains 5.65 acres.
The property has approximately 586 feet of frontage
along William D. Tate Avenue and 283 feet of

REQUESTED CONDITIONAL USE AND COMMENTS:
The applicant is requesting a conditional use permit to amend the previously approved site
plan of CU06-29 (Ord. 06-54) for a personal care facility and long-term acute care hospital,
specifically to modify the building elevations.
The applicant proposes revising the building elevations for the 60 -bed facility by adding
stucco, decreasing the amount of stone veneer and eliminating architectural metal mesh
shading. The proposed additional stucco will be made possible by reconfiguring windows
and using stucco in place of stone.
PRESENT ZONING AND USE:
The property is currently zoned "CC", Community Commercial District and is under
construction.

HISTORY OF TRACT AND SURROUNDING AREA:
The subject property and the property to the north were rezoned from "R-1" Single Family
District to "PCD" Planned Commerce Development District in the 1984 Citywide rezoning.
The site was rezoned to "CC" Community Commercial District in June 1998 (Case No. Z98-
11). The property to the south was rezoned from "C-2" Community Business District to
"CN" Neighborhood Commercial District and "HCO" Hotel Corporate Office District in the
1984 Citywide rezoning. This property was later rezoned to "CC" Community Commercial
District (Case No. Z91-12). The property to the east was rezoned from "R-1 " Single Family
District to "PCD" Planned Commerce Development District in the 1984 Citywide rezoning.
The Unity Church of Northeast Tarrant County was subsequently developed on the site.
The property to the west is William D. Tate Avenue. At the February 17, 2004 meeting
Council approved a conditional use permit (CU03-52) on the subject property for a 60 -bed
personal care facility and long-term acute care hospital. A subsequent revision (CU05-32)
was approved by the Site Plan Review Committee at their May 25, 2005 meeting relative to
modifications to the driveway and parking layout. At the August 15, 2006 meeting, Council
approved a conditional use permit (CU06-29) on the subject property revising the approved
site plan for a two story, 60 -bed personal care facility and long term acute care hospital.

This letter is to verify that your request for a conditional use permit to amend the previously
approved site plan of CU06-29 (Ord. 06-54) for a personal care facility and long-term acute care
hospital, specifically to modify the building elevations on property located at 4201 William D Tate
Avenue and platted as Lot 1, Block 1, 121 Medical Addition was approved by the City Council on
August 21, 2007. A copy of Ordinances 2007-44 and a copy of the approved site plan are
enclosed.
On August 21, 2007, the Planning and Zoning Commission recommended the City Council approve
the request.
Any changes to a site plan approved with a conditional use or a special use (no matter how
minor or major) can only be approved by city council through the public hearing process.
No Conditional Use Permit shall be valid for a period longer than one (1) year from the date on
which the City Council grants the Conditional Use, unless within such one (1) year period: (1) a
Building Permit is obtained and the erection or alteration of a structure is started, or (2) an
Occupancy Permit is obtained and a use commenced.

A request is being made to make minor modifications to the exterior elevations on the
Long Term Acute Care Hospital located at 4201 William D. Tate.
Following is a list of the revisions to the exterior elevations:
1.) Metal panels have been removed and replaced with stone and stucco.
2) 2°d story lobby windows have been removed and replaced with stucco.
3.) Cultured stone has been added around the entry.
4.) Spandrel glass has been eliminated and replaced with stucco.
These changes were made for energy conservation and to ensure the building blends in
nicely with the surrounding structures.
