UES Report No. F241078-1481756
UES arrived on site to perform a Visual Structural Steel Inspection for the above referenced project. Results
of the inspection are as follows:
I Welder Qualifications were reviewed for compliance AWS D1.1
II Joist Girder to Column Welded Connections, Joist Girder to Panel Welded Connections and Joist to
Joist Girder Welded Connections - lines A to F & 19 to 13 – sheets S2.02 & S2.01
A Re-inspected item 1(B) from report 1477185 referencing the joist to joist girder welded
connections. Reference details 211/S5.01 and 209/S5.01. Bolts were not installed in slotted
holes in the joist seats. Per the ERECTOR NOTE on joist drawing sheet J1.0, bolts are
required in joist seats with slotted holes. When bolts cannot be installed, or are removed,
additional weld must be placed in slotted holes. At re-inspected locations - additional welds
have been added at the joist seats as required. This clears item 1(B) from report 1477185.
B Joist girder to column welded connections in the lines 14 to 18 & B to E were inspected per
detail 209/S5.01. At inspected locations, the joist girder to column connections appeared to
be per detail and visually acceptable.
C Joist girder to panel connections in lines 19 & B to E were inspected per detail 207/S5.01.
Inspected joist girder to panel embed appeared to be completed per detail and visually
acceptable.
NOTE(1): The ¾”x6”x6” joist girder stabilizer plates – lines 19 & B to E were not installed at
time of inspection. See detail 207/S5.01
NOTE(2): The welding of the deck support angles in lines 19 & A to F, lines 19 to 13 & F and
lines A & 13 to 19 was incomplete at time of inspection. Work was in progress. See details
202/S5.01, 203/S5.01, 205/S5.01, 213/S5.01 and 214/S5.01
NOTE(3): The bridging terminations along line 19 were incomplete at time of inspection.
Reference New Millennium sheet J2.3 and detail 202/S5.01.
III Precast Panel to Foundation Welded Connections – lines F to A & 1, lines A & 1 to 19, lines 19 & A
to F and lines F & 1 to 19 - sheets S1.01 & S1.02
A Connections were inspected/re-inspected per details 103/S4.01 and 101/S4.01. Panel
P101 was missing a connection plate on one side. The pier embed is misaligned and
this connection will require an RFI. Panel 92 – lines 1&A – is missing (1) of (2)
connections plates. It was noted on 10/14/24 that the connections plates for panels
P24, P23, P25 and P51 have been spliced. The spliced connection plates at panel P23
have been addressed. The spliced connections plates for panels P24, P25 and P51
still require corrective action. The non-compliant welds for the connections plate at
panel P3 have been repaired. The missing connections plates for panel P11 and P12
(noted on report 1477185) have been added. See item 2(A) on report 1477185.

IV HSS(10”x10) Colum to Column Sleeve Welded Connections – lines 8 to 18 & A to F – sheets S1.01
and S1.02
A Inspected connections in the referenced lines have been completed per the contract drawing
and details. Reference details 102/S4.01, 10/S0.04 and detail “A” on sheet AR102.
V Joist To Precast Panel Welded Connections - lines A to F & 19 to 13 – sheets S2.02 & S2.01 – item
1(A) – item 1(A) – Report 1477185
A Along lines F & 18 to 13 – the joist to joist seat connections were inspected per detail
205/S5.01. Reference erection detail 2/E3002. The inspected joists were not fully bearing on
the joist seats at the precast panel along line F. Gaps were visible. Per RFI #33 – “The
reaction point of the joist is at the vertical peg. If the required end anchorage welds can be
achieved in that area of the seat, the gap acceptable. Otherwise, a shim plate can be used
if necessary.” RFI response is attached to this report.*It was noted that bolts were not
installed in slotted holes of the joist seats. Per the ERECTOR NOTE on the joist
drawing, bolts are required in joist seats with slotted holes. When bolts cannot be
installed, or are removed, additional weld must be placed in slotted holes. See
attached detail.
VI Joist To Precast Panel Welded Connections – Panel P34 & P34 – sheets S2.02 & S2.01 – item1(C)
– item 1(C) – Report 1477185
A The joist seat to precast panel at panel joist connection at panels P35 & P34 – was
incomplete at time of inspection. The seat was not welded, and anchor bolts were not
installed. Slotted holes in the joist seat hit at the panel joint. See detail 206/S5.01.
* Discrepancies, if any, are noted above in bold.
