Mr. Jackson:
Back in April, Ron Stombaugh with the City of Grapevine sent a letter to the Corps of Engineers regarding the Club House
expansion at Cowboy's Golf Club.
The permit is ready to be issued through the City, but we have not received the permit from the Corps of Engineers.
Can you please forward us information regarding the Corps permit that Alliance was pursuing for the expansion?

-A full sanitary system cannot be shown due to the existing plumbing being
hidden inside walls and above inaccessible ceilings. To the extent possible with
a renovation project, the requested information is shown on the plans. No other
areas of the building are affected and the existing kitchen (not in scope) has its
own grease interceptor. All new floor sinks at the new bar drain to the new
mechanical grease interceptor located on the cart storage floor with the size
clearly noted on P2.03

DFVV Environmental Group (DFVV) inspected the building (the Site) etthe above -referenced location for
asbestos containing building materials (4[BM) on March 31' 2015. Texas Department of State Health
Services (DSHS)-licensed Individual Asbestos Consultant Robert Friedel (Uc. #105504) conducted this
inspection under an agreement between DFVV and Alliance CM.
The purpose of the studv was threefold: l) determine locations and conditions of asbestos -containing
products at the Site� 2) determine the impact these materials will have on scheduled renovations; and B
assist the owner inobtaining the necessary building permit inaccordance with Senate Bill 5O9�
All accessible suspect homogenous building materials within the defined scope of work area were
inspected to determine the location, condition, and quantity of material. |n compliance with the Texas
Asbestos Health Protectium Rules (TAHPR), a minimum of three bulk samples were collected for each
homogeneous suspect material. DFVVcollected atotal of24samples consisting ofthe following suspect

All bulk samples were submitted for polarized light microscopy (PLM) analysis. The laboratory report in
Appendix A show the results of PLM analysis for bulk samples collected at the site. The laboratory report
shows the building material sampled, provides a description of the material, and analytical results. Based
on the laboratory analysis of the building material samples, asbestos fibers were not identified in the
samples submitted to the laboratory.
Any product containing more than 1% fibrous asbestos, when analyzed by PLM, is considered a potential
hazard by the Environmental Protection Agency (EPA). PLM is the EPA -recognized method for
determining fibrous bulk asbestos content. All samples collected by DFW were analyzed by Hayes
Microbial Consulting DSHS-licensed and NVLAP-accredited laboratory in Midlothian, Virginia. Sample
analysis reported as "None Detected" should be interpreted to mean asbestos was not observed in the
suspect material above the reliable limit of detection, (1% by visual estimation) for the PLM method.
Based on observations made at the Site, and results of the analysis of bulk samples collected during the
inspections, DFW recommends the following:
1. If necessary, collect additional bulk samples from previously concealed materials that may be
revealed during demolition activity, or from materials outside the original inspection scope of work.
"Suspect" material includes any material serving as a sprayed -on or troweled -on acoustic or
fireproofing surface; floor and ceiling tiles; transite panels, siding or shingles; thermal insulation or
any material associated with mechanical systems; or any binding agent such as tar sealant, mastic
adhesive, roofing tar, caulking, et cetera. DSHS regulations require bulk samples in public buildings
be collected by a licensed asbestos inspector or individual asbestos consultant.
2. if any material cannot be positively identified as non -asbestos -containing by the analytical results of
samples collected during this inspection, it should be treated as asbestos until a sample of the newly
identified suspect material can be collected and analyzed by PLM.
3. Following renovation activities, the owner should obtain Material Safety Data Sheets (MSDS) on all
installed building materials. The sheets may be reviewed by an Individual Asbestos Consultant and a
document prepared which could prevent additional testing in the future prior to
renovation/demolition activities.
DFW has endeavored to inspect the existing conditions within the affected areas using TAHPR inspection
protocols. Regardless of the thoroughness of an inspection, it is possible that some areas containing
asbestos were overlooked or inaccessible. This report presents general descriptions of various construction
materials and the general locations where these materials were encountered. If questions arise during the
planning for renovation, demolition or other construction, DFW should be notified to permit us to review
the situation and present recommendations.

I am writing in response to your request to make renovations and improvements to
existing facilities within the Cowboys Golf Club Sublease. Renovations will include the
board room, banquet hall and creating an ADA restroom, In addition, the existing patio
will be replaced and expanded with an additional 1900 square feet outdoor area. Your
request is approved subject to the following conditions:
a. The renovations and improvements are limited to the existing approved main
clubhouse structure, according to the plans and drawings submitted (see attached
