Foundation Repair of Texas for 29 concrete poured sonotubes with steel and 130' of triple 2x8 beams, reset and re -shim
with steel shims. Crawl Space Inspection Report and three drawings sealed and signed by you were submitted by
Bimmerel Construction with addition of second floor in the attic. The foundation drawings submitted for foundation
repair work required to be engineered drawings. I requested applicant to provide engineered drawings but I am still
waiting for it. If you help in this matter I appreciate.

Regarding. 405 Smith Street, Grapevine, TX 76051
Inspection conditions: This Limited Inspection was restricted by extremely muddy conditions in the
crawl space and by having sprayed insulation covering the wood framing. There are also flex type air ducts in
the crawl space that further obstruct access. This inspection report is not an exhaustive list of defects, existing
or potential and should not be construed as a "board by board" identification of damaged wooden members
within the crawl space. This inspection is limited to the original house floor framing and not that of the
addition.
1. Crawl space:
0 Extremely muddy conditions. Drainage corrections should be made.
* Gas could be smelled at one connection. The gas systems should be tested for leaks and repaired as
necessary.
• Significant amounts of trash and debris in the crawl space should be removed.
• The house is supported by block and base supports. Some of the support pads are not centered to the
blocks and need to be corrected.
0 The shims at the original beams are under -sized. These shims should be replaced with shims that fully
span the width of the beam at each location.
0 Some of the original beams are now not supported due to how the last contractor performed shim work.
The shims were installed under the added beams but not under the original beams.
0 The original form boards at the front porch should be removed,
2. Beams: The original beams around the perimeter and under the interior walls are 6"x8" in size. Down
the middle of the house the beam is 6"x8" with a 2"0" ledger on both sides. At the kitchen wall there is a
6"x6" original beam with no support blocks under it. There are several other newer added double 2"x6" beams.
One of these newer beams is twisting over and needs to be corrected, Another of these newer beams needs to
have the end support corrected,
3. Joists: Are 2"x8" members spaced on 19 inches. The joists under the front dining room run from the
front to the back of the house. The joists everywhere else run from side to side, No joist recommendations.

Good Afternoon: PERMIT EXRIRAT�ON,
Crosstown Engineering (CE) was retained to inspect the subject foundation and to provide an opinion re- U U)
garding the performance of the foundation and opine as to whether or not Perma-Pier Foundation Repair Z
of Texas (The Contractor) has prepared a reasonable limited repair plan. This report has been prepared in W,
accordance with the requirements of a "Level B" survey as defined by the Texas Chapter of the American Z
Society of Civil Engineers and the Texas Board of Professional Engineers. 0
This report provides our reasonable professional opinion of the condition of the foundation on the date of 0
our inspection and does not take into consideration any changes in the condition of the foundation or soils
after that date. The contents of this report supersede any verbal comments made regarding the structure i.
before, during or after the inspection.
Scope of Work Limitations:
This report is for informational purposes only and is not intended to provide a detailed inventory of de-
fects or a technical evaluation of the property. The inspection excludes the framed superstructure, de-
tached buildings, privacy or retaining walls, general site drainage away from the structure, material and
soil sampling/testing, and verification of concrete reinforcement or knowledge of the location of interior
grade beams, boxed structural members not in plain sight or previous repair work. Removal of floor cov-
erings or performance of invasive tests or procedures is not included.
Document Review:
The Contractor provided an agreement and a limited repair plan that included the pier locations and pre -
lift elevations for the structure for our review. No other information was provided regarding prior engi-
neering reports, recommendations for foundation repairs or construction documents relative to this struc-
ture.
General Observations:
For the purposes of this report our directions will be described using the terms left, right, front and back
with the front referring to the side of the structure with the front door.
The structure is two stories with a pier and beam foundation. The primary structural system of the struc-
ture is a wood framed system with exterior wood panelling and interior drywall with various finishes.
The foundation was not exposed.

Grading, Drainage, Erosion and Vegetation Observations:
The terrain immediately surrounding the structure was visually observed during the inspection. We ob-
served the following:
• A full perimeter functioning gutter system was not installed.
• A functioning foundation watering system was not in place.
• The grading on-site was generally flat or sloped away from the foundation.
• The terrain is landscaped with grass, several trees, and some shrubbery. Some trees and shrubs
are close to the foundation.
• There are large trees near the foundation with limbs overhanging the roofline.
Floor Elevation Discussion:
A relative elevation floor survey was performed using a Ziplevel Pro -200013 to map the surface topogra-
phy of the floor(s) of the living area and garage. The floor plan(s) and the elevations are illustrated in the
attached Site Map. The elevations were adjusted based on the flooring type encountered to be on the
same plane as the base point floor type. The garage ceiling (if a garage was present) was measured and
adjusted to be on the same plane as the foundation. Garage floor are designed to slope and are not as ef-
fective in measuring foundation movement.
Visual Observations:
Minor damages were observed. Based on our observations of the structure, the structure's physical dam-
age, and review of the elevation map, we believe the structure has experienced foundation movement over
the life of the structure, resulting in differential movement of the slab on grade foundation.
Foundation Repair Recommendations:
Perma-Pier Foundation Repair of Texas has recommended installation of 29 concrete poured sonotuebs
with steel, 130 LF of triple 2x8 beam, and to reset/reshim with steel. The repairs are to be made in the
approximate locations of the structure as indicated on the attached limited repair plan. CE recommends
evaluating the drainage at a future date.
Upon review of the elevation drawing and based on the contractors evaluation of the property and our
visual inspection and verified elevation map, we believe the proposed limited repair plan to be a reasona-
ble solution. Additional replacement of the crawlspace elements of the foundation may be needed during
the repair process because deteriorated wood is not easily identified upon visual observation. This does
not rule out the possibility of an additional limited foundation repair plan in the future should damages or
foundation movement worsen or change. Re-evaluation of the structure in the future may be prudent.
The purpose of the installations is to support the failed areas and improve elevations in areas of differen-
tial settlement. Please see the limited repair plan for more information.
A foundation and yard watering and foundation maintenance program will help control seasonal move-
ment. It is important that the soils be stabilized and maintained with grass or ground cover around the
perimeter of the structure. In addition, tree root barriers for trees near the foundation or with limbs that
grow near or overhang the roofline and vegetation maintenance around the foundation is also advisable.

A fully functioning gutter system, which directs discharge through downspouts away from the foundation,
is also advisable.
We recommend pre and post -lift plumbing tests be performed on the sewer and potable plumbing lines.
The results of the tests should be immediately provided to our office.
Expectations:
Because the structure has endured pre -repair foundation differential movement, residual differential ele-
vation and perceptible floor slope or some leaning door frames/windows or other noticeable issues may
remain following the foundation repairs.
The soils beneath and surrounding the structure are known to shrink and swell as the seasonal soil mois-
ture content fluctuates. Moving forward, we anticipate that some cracks in the interior and exterior walls
will surface due to seasonal movement within the soils, even after foundation repair. Periodic repair of
this type of cracking may be needed.

It is known to knowledgeable professional engineers that the soils in this area are subject to movement due to expan-
sion, contraction or densification of the soils etc. This soil movement could possibly cause the foundation to move
after the remediation plan within the attached report has been implemented and may impact the stability of the foun-
dation and cause damage.
NO WARRANTY IS EXPRESSED OR IMPLIED BY THIS ENGINEER AS TO THE PERFORMANCE OF THIS
FOUNDATION OR THE REPAIRS THERETO. Diligent foundation maintenance to maintain consistent soil con-
ditions along the perimeter should reduce further problems after the recommendations within this report have been
implemented. However, seasonal moisture variations, water leaks, erosion and other factors may affect the stability
of the foundation and put it in danger of further damage.

property or an inventory of defects. The opinions expressed in this report are based on a visual evaluation of current
conditions observed at the time of the inspection. THERE IS NO WARRANTY, EXPRESSED OR IMPLIED,
CONCERNING THIS ENGINEERING REPORT.
The information in this report supersedes any verbal comments, expressed or implied, made by Crosstown Land
Development Services or its principals, agents or employees. The client agrees that CLDS nor its employees or
owners will be responsible for:
1. Knowledge of the subsurface conditions without extensive geotechnical data obtained from onsite drill-
ing and testing of the recovered samples,
2. Knowledge of cracks, vertical differential displacement of floors without uncovering of the floor by the

Perma-Pier Foundation Repair of Texas (The Contractor) retained Crosstown Engineering (CE) to evaluate
the installation data in conjunction with limited foundation repairs proposed by The Contractor. All
opinions provided in this report are based solely on our review of provided documentation and not by visual
inspection by CE.
29 concrete poured sonotubes with steel and 130 LF of triple 2x8 beams have been installed and the
structure was reset and re -shimmed with steel according to The Contractor. To the best of our knowledge
and based on document review only, The Contractor has performed the limited repairs in substantial
accordance with their provided agreement and limited repair plan dated 3/4/16 and our initial consultation
report dated 5/3/16, including any approved changes.
If visual verification of limited repairs is desired, please contact our office to schedule a visual inspection
appointment with one of our inspectors for a small fee. Additional limited repairs may be needed upon our
visual inspection.
A foundation and yard watering and foundation maintenance program will help control seasonal movement.
It is important that the soils be stabilized and maintained with grass or ground cover around the perimeter
of the structure. In addition, tree and shrub removal or root barrier installations are advisable if trees or
bushes are observed to be close to the foundation. Tree roots can cause "heave" or settlement of the
foundation where roots grow and absorb moisture over time when planted close to the foundation (tree roots
extend approximately as far or further than tree limbs).
We also recommend re -grading the soils around the structure to slope away from the foundation to prevent
excessive ponding near the foundation in areas that are not already sloping away. In addition, we
recommend that a full gutter system with gutter extensions be installed to discharge a minimum of 4 feet
from the foundation (not in landscaped flower beds or any other location that will trap water near the
foundation). Ponding near the foundation can cause "heaving" (upward movement of the foundation),
settlement of the foundation due to overly saturated clays soil or erosion of soil embedment around the
foundation.
If the owner, at any time, observes standing water near the foundation, we recommend that they consult
with an engineer or a drainage contractor immediately.
We recommend pre and post -lift plumbing tests be performed on the sewer and potable plumbing lines.

Observations:
Based on the observations made by The Contractor, and our observations of the pre -repair foundation
elevations/observations provided by the Contractor, the structure has experienced foundation movement
over the life of the structure.
Expectations:
Because the structure has endured pre -repair foundation differential movement, residual differential
elevation and other cosmetic issues may remain such as door sticking and doorway leaning following the
limited foundation repairs.
The soils beneath and surrounding the structure are known to shrink and swell as the seasonal soil moisture
content fluctuates. Moving forward, we anticipate that some cracks in the interior and exterior walls will
surface due to seasonal movement within the soils, even after foundation repair. Periodic repair of this type
of cracking will be required.
Scope of Work Limitations:
Crosstown Engineering has provided a review of the provided documentation only and has not been retained
to provide a visual inspection of the property. This report is for informational purposes only and is not
intended to provide a detailed inventory of defects or a technical evaluation of the property. The

Crosstown Engineering (CE) was retained to inspect the subject foundation and to provide an opinion re-
garding the performance of the foundation and opine as to whether or not Perma-Pier Foundation Repair

Scope of Work Limitations:
This report is for informational purposes only and is not intended to provide a detailed inventory of de-
fects or a technical evaluation of the property. The inspection excludes the framed superstructure, de-
tached buildings, privacy or retaining walls, general site drainage away from the structure, material and
soil sampling/testing, and verification of concrete reinforcement or knowledge of the location of interior
grade beams, boxed structural members not in plain sight or previous repair work. Removal of floor cov-
erings or performance of invasive tests or procedures is not included.
Document Review:
The Contractor provided an agreement and a limited repair plan that included the pier locations and pre- d
lift elevations for the structure for our review. No other information was provided regarding prior engi-
neering reports, recommendations for foundation repairs or construction documents relative to this struc-
ture.
General Observations:
For the purposes of this report our directions will be described using the terms left, right, front and back
with the front referring to the side of the structure with the front door.
The structure is two stories with a pier and beam foundation. The primary structural system of the struc-
ture is a wood framed system with exterior wood panelling and interior drywall with various finishes.
The foundation was not exposed.

Grading, Drainage, Erosion and Vegetation Observations
The terrain immediately surrounding the structure was visually observed during the inspection. We ob-
served the following:
• A full perimeter functioning gutter system was not installed.
• A functioning foundation watering system was not in place.
• The grading on-site was generally flat or sloped away from the foundation.
• The terrain is landscaped with grass, several trees, and some shrubbery. Some trees and shrubs
are close to the foundation.
• There are large trees near the foundation with limbs overhanging the roofline.
Floor Elevation Discussion:
A relative elevation floor survey was performed using a Ziplevel Pro -200013 to map the surface topogra-
phy of the floor(s) of the living area and garage. The floor plan(s) and the elevations are illustrated in the
attached Site Map. The elevations were adjusted based on the flooring type encountered to be on the
same plane as the base point floor type. The garage ceiling (if a garage was present) was measured and
adjusted to be on the same plane as the foundation. Garage floor are designed to slope and are not as ef-
fective in measuring foundation movement.
Visual Observations:
Minor damages were observed. Based on our observations of the structure, the structure's physical dam-
age, and review of the elevation map, we believe the structure has experienced foundation movement over
the life of the structure, resulting in differential movement of the slab on grade foundation.
Foundation Repair Recommendations:
Perma-Pier Foundation Repair of Texas has recommended installation of 29 concrete poured sonotuebs
with steel, 130 LF of triple 2x8 beam, and to reset/reshim with steel. The repairs are to be made in the
approximate locations of the structure as indicated on the attached limited repair plan. CE recommends
evaluating the drainage at a future date.
Upon review of the elevation drawing and based on the contractors evaluation of the property and our
visual inspection and verified elevation map, we believe the proposed limited repair plan to be a reasona-
ble solution. Additional replacement of the crawlspace elements of the foundation may be needed during
the repair process because deteriorated wood is not easily identified upon visual observation. This does
not rule out the possibility of an additional limited foundation repair plan in the fixture should damages or
foundation movement worsen or change. Re-evaluation of the structure in the future may be prudent.
The purpose of the installations is to support the failed areas and improve elevations in areas of differen-
tial settlement. Please see the limited repair plan for more information.
A foundation and yard watering and foundation maintenance program will help control seasonal move-
ment. It is important that the soils be stabilized and maintained with grass or ground cover around the
perimeter of the structure. In addition, tree root barriers for trees near the foundation or with limbs that
grow near or overhang the roofline and vegetation maintenance around the foundation is also advisable.

A fully functioning gutter system, which directs discharge through downspouts away from the foundation,
is also advisable.
We recommend pre and post -lift plumbing tests be performed on the sewer and potable plumbing lines.
The results of the tests should be immediately provided to our office.
Expectations:
Because the structure has endured pre -repair foundation differential movement, residual differential ele-
vation and perceptible floor slope or some leaning door frames/windows or other noticeable issues may
remain following the foundation repairs.
The soils beneath and surrounding the structure are known to shrink and swell as the seasonal soil mois-
ture content fluctuates. Moving forward, we anticipate that some cracks in the interior and exterior walls
will surface due to seasonal movement within the soils, even after foundation repair. Periodic repair of
this type of cracking may be needed.
