This report constitutes the engineering opinion that you requested on the foundation at the
referenced address. It provides the professional opinion of the writer based upon visual
observations of property features and damages that could be observed without removing
any obstacles and without taking soil samples or performing plumbing testing or any type
ofdestructive testing_ This report includes visual observations of features and damages and
measurements of floor elevations that- were taken October 29,2012. Recommendations for
corrective actions that are advisable at this time are made based upon the observations and
measurements on that date. This report is basically intended as a "Level B" evaluation as
defined by the Texas Board of Professional Engineers but may not contain all elements of
the definition. Unless specifically mentioned, it excludes the framed superstructure,
detached buildings, any verification ofconstructionto building Codes, general site drainage
except in the immediate vicinity of the foundation, and privacy or retaining walls. You
should note the "Agreements and Limitations" section at the end of this report.
OBSERVATIONS
This building is a one story wood framed structure with brick veneer and wood siding. The
foundation is a concrete slab and is over 20 years old. For orientation purposes and
discussion in this report, the building faces South.
A. Exterior distress symptoms and observations.
• S3 -stairstep crack in brick and mortar to 1/4' wide
• G4- wood to brick separation greater than 1/4" -wide.
• F2- crack in perimeter beam to 118" wide.
• TP- nim gapped apart at joints.
Soil condition- Soil slope incorrect & low at positions marked "L". Slope on the
East side incorrect - drains exist on this side but are higher than close to the house.
Lot slopes towards the street.

B. Interior Observations:
• Not all distress is shown. Doors at positions "A" are off level and low towards the
low elevations.
• Plaster cracks in the vv all at positions `B 1". Small wall cracks in other rooms also.
Crack in drywall in the ceiling at positions "C".
Top of bathtub off level at position "7'.
Walls lean out at locations "1". Tile cracks at :T".
Repaired incoming plumbing leak in the family room.
Large repaired cracks in the floor of garage.
C. Floor elevations of the interior were taken using a Stanley "CompuleveP'. Elevations
were recorded to the nearest .l inch in numerous locations as shown on the attached plan
view. Elevations of the porch and garage are normally not taken because porches and
garages are normally constructed to have significant slopes. However garage elevations are
shown for reference. An arbitrary 0.0 benchmark was taken in the hall. Elevation
differentials varied from a high point of =0.0" at the baseline to a low of -2.7" in the master
bedroom. The total differential measured at this time is therefore 2.7 inches. It should be
noted that the elevation measurements include effects of elevation variation in the original
construction.
CONCI.LESIO\S
The total elevation variation is beyond the limits of generally accepted guidelines for a
house of this size; age. and type. The house has shown significant settlement towards the
outside perimeter on all sides. This settlement has caused most of the various distress
symptoms.
Because of the wide variation in elevations there is a high probability that there is a new
plumbing leak. This should be checked. However the rear settlement may well have been
caused by the past incoming leak.
Because of the summation of internal and external distress factors it is recommended that
17 piers be added as shown by the attached plan.
Additional improvement should be obtained through implementing the `Foundation
Maintenance Program".

RECONL'VIENDATIONS
1) Watering Program: Watering details are described in the attached "Foundation
Maintenance Program".
2) Water and Sewer: Conduct water pressure and seNver gravity tests on the house to
determine if there are any water or sewer leaks. This work is always recommended after
foundation work and is suggested but not required before foundation work. This could
establish the basis for possible filing of insurance claims. Any leaks detected should be
repaired immediately=.
3) Drainage: Use the guidelines in the attached "Foundation Maintenance PIan" to define
correct soil levels and slope. Utilize any excess soil from the pier work to eliminate low
areas next to the foundationto improve water flow away- from the foundation and to prevent
water from ponding against the foundation. Add a "clayey" type of soil as needed to obtain
proper soil levels as described in the "Foundation Maintenance Program." Direct water
away from the foundation. As a general rule of correct drainage; one hour after a hard rain
there should be no water standing within 15 feet of the foundation.
4) Piers: Install a minimum of 17 piers as shown by the plan. See the "Agreements and
Limitations" for qualifying information. The foundation should be lifted using these piers
as much as practical to improve the level of the foundation without causing undue collateral
damage to the interior or the exterior of the house.
Pier Specifications: Piers to be constructed using pressed pilings of concrete or steel.
Pressed pilings are to be pressed to refusal. The pilings must include an acceptable
interlocking technique. Excavations shall be filled with clay soil and compacted to prevent
penetration of excessive surface moisture. The foundation company should provide a
suitable warranty to guarantee the piers against vertical settlement.
Expected Results: The repair work is intended to reduce the amount of deflection in
foundation and floor surfaces, improving distortion in the immediate area (within generally
a maximum of 8 feet) of the repairs. Lifting should be done in a manner to obtain as much
improvement as practical without causing undue damage from the lifting operations. The
repairs do not eliminate the need for additional repair work in the future and are not
represented to "stabilize" the foundation or be an all inclusive remedy. All portions of the
house are subject to future differential movement due to variations in soil moisture and /or
other factors. It should be understood that damage to plumbing systems and cracking of
rigid surfaces such as masonry, concrete, and drywall surface can result from lifting and
void filling operations.

AGREEMENTS AND LINUTATIONS
Use of this report for any reason implies consent to all agreements and limitations of this
report. Maximum liability of Barry kockler, P.E. and/or Devtek is limited to the cost of
this report. Any legal disputes to be settled via arbitration. This report is a presentation of
professional opinion based upon a limited evaluation of the property that includes a visual
examination of the exposed interior and exterior finishes of the structure and the ground
surfaces adjacent to the foundation and to taking of relative floor elevations. We did not
inspect areas that were physically inaccessible; unsafe, or covered by obstacles. The taking
and testing of soil samples was not included. Testing for plumbing leaks was not done but
is recommended that a plumbing leak inspection be done prior to and after doing any
repairs. Roof framing was not inspected unless specifically requested and then only in the
vicinity of the access hole. A termite inspection was not performed. De-,rtek does not test
for mold. Listed defects are not meant to be all inclusive but are included as a general
indication of conditions at the time of the observations and measurements. "Devtek"
cannot be responsible for knowledge of concealed cracks or other distress ofany type in any
other structural elements such as joists, beams, or slabs.
Included recommendations are only a professional opinion based upon measurements and
observations taken at the time indicated on the report. In particular, elevations can vary,
primarily dependent upon soil moisture conditions at the time of measurements. Included
recommendations are only a professional opinion and do not represent a warranty or
performance contract with anyone. Information on this report supersedes any verbal
comments made by the Engineer and/or any other Devtek Engineering Co. representative.
This evaluation is approximately described as a Class "B" study but may not exactly meet
all the terms in the definition by the Texas Board of Professional Engineers.
There is some level of risk with all foundations. dote that all foundations built on
expansive soil will experience some degree of movement as the soil moisture content
varies. Unless specified in the report, we had no access to the original construction of the
building or to witness any previous repairs. We make no statements as to structural quality
and do not predict future performance of the foundation. The professional opinions
expressed in the report are based upon normally accepted construction practices and quality
control having been followed. Determination of constructionto Building Code is best done
during the original construction and is not part of this evaluation.
In general, pier performance improves as its depth increases. In this area, the depth
required to obtain satisfactory performance of a pier varies widely. This depth can be less
if the pier encounters substantial rock. The recommended design is to serve as a minimum
standard that is sen without the benefit of a site specific soil investigation, which is not part
of this work. If the recommended repairs are not completed within 90 days, or if new
distress is noted, the foundation should be reevaluated.

The repair work specified in this report is designed to improve the degree of deflections
noted in this report_ It is remedial in nature and will not recover all lost elevation.
Typically, a pier will improve slab elevations for a maximum of 8 feet from the pier. The
house may still be out of level beyond normal tolerances after repairs. A pier will not
prevent upheaval. If upheavals are present, elevation improvements maybe minimal. We
cannot be responsible for the foundation repair or for future satisfaction with the
foundation. The repairs do not eliminate the need for additional repair work in the future
and are not represented to "stabilize" the foundation or be an all inclusive remedy. Future
adjustments of the piers may be needed. The level ofthe house may still be beyond normal
tolerances after repairs. Dev-tek does not offer or imply any warranty° for the repairs or for
the repair company acts or omissions or for any other person conducting the repairs. It is
hi,hly recommended that you obtain a suitable warranty from the repair company. Devtek
does not imply future ability of any repair company to honor its warranty. Selection of a
repair company is the responsibility of the user of this report.
Please be advised that additional report consultation is available at additional cost.
Because maintenance of uniform soil moisture is important to achieving best performance
from a slab on grade foundation, a recommended "Foundation '-Maintenance Program" is
included with this report. If plumbing leaks are resolved or drainage improvements are
made it will take several months for the soil to reach a new equilibrium. During that time,
the differential movement of the slab could be larger than the normal movement caused by
seasonal moisture variation.
This report is intended for use of the addressee only and not for any other person, firm, or
corporation. Neither is it for usage by the owner of the property that is the subject of this
report unless the owner is the addressee. Devtek has no contractual arrangement with any
party other than the addressee of this report.
It is the responsibility of the foundation repair company to insure that piers are
driven to refusal and to provide a warranty. This report does not warrantor predict
the future performance of the subject foundation.

Foundation Maintenance Pro6ram
Note; The following are General comments about foundation maintenance and do not override
specific recommendations of this report.
Soils found in the north Texas area typically contain a high percentage of active clays. These exhibit
a high degree of expansion as the moisture content increases and shrinkage as the moisture content
decreases. Variation in the moisture content of the soil under the foundation can cause differential
movements of the foundation and result in structural problems. Consistent soil moisture control is
therefore a key to controlling foundation related problems. The chances of foundation distress and
failure are reduced when the moisture content of the active supporting soil layer at the perimeter of
the foundation is maintained uniformly.
Drainage
Soil levels should be a minimum of 2 inches below the bottom of wood or brick for a "slab on grade"
type of foundation. The soil should be sloped away from the foundation at a rate of approximately
one inch per foot for a minimum of four feet away- from the perimeter. Soil near the foundation
should be predominately a clay material and not a sandy loam or other porous material. Water
should not stand within 15 feet of the foundation for one hour after a heat- rain. Gutters are not
required but can be an aid to improved drainage, particularly if planters are near the foundation.
Downspouts should discharge away from the foundation and at a distance of at least 3 feet from the
perimeter.
Trees and Shrubbery
Trees and shrubbery° can have an adverse effect on the foundation and also cause problems with
sidewalks, driveways; and patios. Their roots can sap moisture from the soil near the perimeter of
the house and under the slab. This lowers the moisture content of the active supporting soil layer
and can cause differential movement of the foundation. Cutting the roots at or near the foundation
and the installation of "root barriers' may be necessary to reduce the adverse effects of aggressive
root systems. In some cases, it may be necessary to remove large trees or shrubs near the foundation.
Watering
A constant soil moisture content all around the house and all seasons of the year is the goal of
foundation maintenance for "slab on grade" foundations. During the dry season sufficient water
should be added to the soil near the slab perimeter to maintain soil moisture. This is best
accomplished by using soaker hoses placed at 18 inches from the foundation. The south and west
sides of the house may require the most water because the sun's drying effect is greater than on the
north and east sides. During the wet season, drainage should be checked to avoid standing water near
the foundation. Planter barriers should be perforated or removed to avoid trapping surface water
near the foundation.

Subject: Compliance Report and Inspection of Remedial Foundation Repairs to:
- 1818 Branch Hollow Lane, Grapevine, Texas 76051
The piers were verified after installation by taking new elevation measurements and by
observations of feature improvements. The piers were observed before holes were covered.
The foundation repair was determined to be within the specifications of Devtek Report No.
32111. New elevations were taken near the added piers and the final inspection was made
after lifting the house.
The company that did the work was Geo -Tech Foundation Repair.
The piers were correctly adjusted. Elevation variation in the main house was reduced
from 2.7" to 1.4". The previous lowest spot was lifted approximately 1.3". The front
of the garage was lifted approximately 0.8". Significant improvement was noted in the
various features. Additional lifting is not recommended. The house is now within normal
standards for elevation variations.
This report does not warrant or predict the future performance of the subject
foundation. It is up to the foundation company to warrant the foundation repair and
insure that the piers are driven to refusal.
