Item #1:
CONCRETE PAVEMENT REPAIR NVITM FLEXBASE: 2,534 Square Feet
Saw cut defined limits of areas. Excavate failed pavement to a depth of 12". Install 4" of crushed concrete
and compact to .a min of 95% density. Under cut edges 3" under existing concrete slab. Drill ,dowel and
epoxy grout edges of repair with #4 deformed dowels. Place 8" concrete with a broom finish. Reinforce
with #4 rebar at 24" on center. Concrete shall be 4,000 PSI with air entrainment for durability and resistance
to surface scaling. Cure concrete with a liquid membrane curing compound to insure strength development.
Saw cut control joints as necessary.

CRACK ROUTING & SEALING FOR CONCRETE PAVEMENT (CRAFCO 34222): 5,000 LF
With mechanical routing tools create a groove about 3/8 inches wide and 3/4 inches deep along the crack to
create a reservoir for the crack sealant. Immediately prior to sealing, remove loose foreign material by
blowing out the crack with compressed air until completely free of dust and water. Material shall be a hot
pour rubber material, capable of effectively sealing cracks in the pavement against the infiltration of surface
moisture.

SEALING EXISTING CONCRETE CONTROL JOINTS (CRAFCO 34222): 3,000 LF
With mechanical saws, clean joints to remove existing sealants and loose foreign material by blowing out the
crack with compressed air until completely free of dust and water. Material shall be a hot pour nibber
material, capable of effectively sealing cracks in the pavement against the infiltration of surface moisture.

Scope of Work - Concrete Repairs
(blends City of Grapevine requirementswith Prologis requirements)
Sawcut defined limits of area. Remove existing curb and pavement to a depth of 8 Inches.
Replace with at least 4 inches of compacted crushed aggregate (or an approved select fill
material), or more if necessary in orderto sustain a stable subgrade Excavate the area to a
depth of 2 inches below the existing concrete slab. Subgrade'depth' will beat least 12 inches in
depth below the existing concrete surface elevation. Hand dig 2 inches in depth and 3 inches
horizontally under the existing concrete slab, surrounding the area of repair. Place 8 inches
concrete with broom finish. Reinforce with #4'rebar at 24inch on center with dowels placedd
into adjacent' concrete on 24 inch intervals. Concrete shall be 5,000 PSI.

1. Contractor shall execute the Services described as:
Perform the work detailed In Exhibit A (32 pages) per proper specifications depending upon the
location of the %property. All work should adhere to Prologis Specifications' (Exhibit C, 5 pages)
unless otherwise noted. All properties located in Grapevine, TX should adhere to the blended City of
Grapevine/Prologis Specification ( Exhibit B,1 page). All properties located in DFW Airport should
adhere to the DFW Airport/Prologis Blended Specifications (Exhibit D, 4 pages). All crack sealing
should utilize Crafco Roadsaver 222 Sealant '(product 'data sheet attached as Exhibit F, i page).: All
work performed should adhere to the guidelines set forth In the original bid invitation letter dated
June 1, 2016 (attached as Exhibit E, 2 pages). The work outlined In Exhibit A shall be performed for
the cost equivalent to the proposal submitted by Asset Pavement via email on June 16 2016. This
cost Is detailed In Exhibit G. Any changes to cost or scope of work shall be mutually agreed upon
and documented with fully executed change orders.

Except as set forth below, this Addendum to Service Agreement, and all attachments hereto, are hereby incorporated
into the Service Agreement between Owner and Contractor referenced above. No general terms or other conditions
submitted by Contractor shall be incorporated into any Service Agreement, despite being attached hereto, unless
expressly referenced in Section 9 above.
In the event of any conflict between this Addendum to Service Agreement, and the terms and conditions of the
Service Agreement, the terms and conditions of this Addendum to Service Agreement shall control.
This Addendum to Service Agreement is entered into as of the day and year first written above.
This Agreement, and any change orders, modifications, amendments, and/or addenda hereto, may be executed in
any number of counterparts, each of which shall be deemed to be an original, and all of such counterparts shall
constitute one Agreement. To facilitate such execution, the parties may execute 'and exchange by email in PDP
format counterparts of the signature pages, which shall be deemed originals.
