1. Contractor shall execute the Services described as:
Per Protogis specifications, contractor shall provide all labor, equipment and tools to perform
concrete replacement at the location stated below (see attached picture known as Exhibit A).
Contractor shall saw cut defined limits of areas. Excavate failed pavement to a depth of 8". Undercut
3" under existing concrete stab. Drill dowel and epoxy grout edges of repair with #4 deformed
dowels, Place 8" concrete with a broom finish. Reinforce with a #4 rebar at 24" on center. Concrete
shall be 3500 PSI air entratnment for durability and resistance to surface scaling. Cure concrete with
a liquid membrane curing compound to Insure strength development Saw cut control joints as
necessary.

Except as set forth below, this Addendum to Service Agreement, and all attachments hereto, are hereby incorporated
into the Service Agreement between Owner and Contractor referenced above. No general terms or other conditions
submitted by Contractor shall be incorporated into any Service Agreement, despite being attached hereto, unless
expressly referenced in Section 9 above,
In the event of any conflict between this Addendum to Service Agreement, and the terms and conditions of the
Service Agreement, the terms and conditions ofthis Addendum to Service Agreement shall control.
This Addendum to Service Agreement is entered into as of the day and year first written above.
This Agreement, and any change orders, modifications, amendments, and/or addenda hereto, may be executed in
any number of counterparts, each of which shall be deemed to be an original, and all of such counterparts shall
constituto one Agreement. To facilitate such execution, the parties may execute and exchange by email in PDF
format counterparts of the signature pages, which shall be deemed originals.

Asset Pavement requires the below outline be incorporated into any contract/ purchase order so that all
inclusions, exclusions, and provisions are defined and understood. This proposal is valid for thirty (30) days.
After thirty (30) days, pricing is subject to changes due to variances in the cost of materials. We appreciate
the opportunity to provide you with this proposal for this project and hope to become part of your construction
team. If you have any questions feel free to call me at your earliest convenience.
Exclusions: sales tax, testing, licenses, bonds, permits, engineering services or fees, removal or handling of
contaminated spoils/hazardous materials, landscaping, removal of asphalt with reflective crack control type
fabric (such as but not limited to petromat), removal/relocation/accidental damage of any underground
obstructions and/or utilities (such as but not limited to private electrical and/or sprinkler lines), sub grade
repair, excavation, grading, or backfill other than described above.
Special Provisions:
1. Asset Pavement will agree to mutually acceptable construction schedule and will not accept responsibility
for liquidated damages due to changes in schedule by other trades or weather delays.
2. Prices have been line itemized for clarity. However, the grand total or combined pricing reflects the entire
scope to take advantage of economies of scale. Reduction in scope may impact pricing.
3. Payment is due within thirty (30) days of the Asset Pavement invoice date. if Asset Pavement is not paid
within thirty (30) days, a late fee will be assessed in the amount of 10% of the unpaid invoice amount.
4. Final payment, if retainage is held, shall be received by Asset Pavement no later than thirty (30) days from
the date of final or retainage invoice. If not paid within thirty (30) day period, a late fee will be assessed in the
amount of 10% of the final or retainage invoice amount; no discount will apply.
5. All work bid is based on being performed during weekday business hours. Monday through Friday 8:00am
to 5:00pm.
