The survey was performed to facilitate upcoming renovation activities. The scope of work for this project was limited to the identification, sampling, and analysis of suspect asbestos -containing materials (i.e., vinyl floor the and associated mastic, ceiling tiles, and covebase and associated mastic) throughout the store. The asbestos survey was conducted by Mr. Art Ruiz on March 25, 2017. Mr. Ruiz is a State of Texas licensed Asbestos Building Inspector. Refer to Attachment I of this report for a copy of Mr. Ruiz's license and certifications. Nine suspect asbestos -containing materials (ACMs) were identified from which a total of 21 samples were collected. ACM is defined by the U.S. EPA and Occupational Safety and Health Administration (OSHA) as any material containing more than 1% asbestos. The samples collected were analyzed by Polarized Light Microscopy (PLM) in accordance with the U.S. EPA "Method for Determination of Asbestos in Bulk Building Materials" 600/M4-82-020 per 40 CFR 763 Method. This method requires that multiple and distinct layers be analyzed separately. General. Refer to the floor plan in Attachment II for the location of samples collected and ACMs identified, and Attachment III for photos of the facility and identified suspect ACMs. Of the nine suspect ACMs tested, none of the materials were found to be asbestos -containing. A summary of the suspect ACMs identified and the analytical results are provided in Table 1.0 on the following page.
