1. A separate permit and 2 sets of drawings are required for each shp . For wall signs a separate permit application and
plans may be submitted for each elevation.
2. Pole or ground signs require a site plan drawn to scale showing location of the sign. Show dimensions and distances to
property lines. Pole sign structural drawings must be sealed by a State of Texas Registered Professional Engineer.
Monument and Ground sign applications must include footing detail.
3. Detailed,dimensioned plans of sign showing graphics.
4. Dimensioned building elevation showing sign or signs on building. Show dimensions of height & width of tenant space
and or building.
5. Plan Review Fee,65%of the permit fee is required when permit application is submitted. Balance due upon approval.

REAL ESTATE SIGN VACANCY AFFIDAVIT
Section 60.B.1.e of the City of Grapevine Comprehensive Zoning Ordinance
requires that an affidavit be submitted for all Real Estate Signs advertising the
lease or rent of buildings or space within buildings. Such affidavit shall certify that
the building is currently vacant or will become vacant within 30 days. The affidavit
shall be submitted by the property owner or licensed property management
company. The affidavit shall not be submitted by the sign contractor.

within 30 days of today's date.
I understand that the permit is valid for a period of not more than one (1) year, and may be
renewed subject to Section 60.13.1.e of the City of Grapevine Comprehensive Zoning Ordinance. I
further understand that violations of the zoning ordinance are subject to penalties up to and
including the issuance of citations. In accordance with Section 70 of the City of Grapevine
Comprehensive Zoning Ordinance anyone found guilty of violations shall be fined a sum not to
exceed 2,000.00 dollars for each offense, and a separate offense shall be deemed committed on
each day during or on which a violation occurs or continues. ,

In reference to above permit,there is no record of Grapevine Medical Village business in the city system.On this lot car
wash building is still existing.There is no permit application submitted to remove existing car wash building neither any
documents submitted for new Grapevine Medical Village building.
Sign permit application shall be filled out completely, provide name of Business on the application form.
Label setback of the sign from the property lines, minimum 10' is the required setback.At the front there is 12 feet
utility easement, 29.95 feet right of way and TX DOT R.O W. and 25 feet mutual ingress and Egress easement (total
54.95').Sign shall not be constructed within any easement.
V shaped sign shall have an angle of 90 degree or less between sign face. Clarify what is"sign type horizontal v-shape"
mean?
Label height of the sign from grade level.

Dear Applicant:
On January 9.. 2018 sign applications were submitted to this office for review for
the above referenced address. Our records indicate that you never responded to
plan review comments. Your permit application has expired. A new application,
plans and plan review fees are required. Please resubmit for your project.
Performing work without a permit is a violation of city ordinance, regardless of
whether or not an application has been made, and could result in the revocation
of your contractor's registration for a period of not less than one year, and up to a
$2,000.00 fine. Additionally, failure to obtain your permit within 180 days of
application has resulted in the expiration of the permit.
