This letter confirms on October 22, 2018 the Historic Preservation Commission staff
approved with conditions #CA18-119 for the property addressed 911 East Worth Street,
legally described as Block 4, Lot 5, D. E. Box Addition, City of Grapevine Texas. The following
was approved on this Certificate of Appropriateness.-

1. Install 14 new exterior concrete pressed piers under existing house;
as per the attached plan with the conditions a permit be obtained from the Building
Department.
An approved Certificate of Appropriateness is not an approved building permit; a
building permit is / is not required. Contact the City of Grapevine's Building
Department at 817.410.3165 for fees and information regarding permits.

Provide transferable lifetime warranty on all IFR installed piers

Note: There is a 5% fee added to the total for all credit card transactions. Complete leveling is not to be expected.
Integrity Foundation Repair is not financially responsible for plumbing issues as a result of the lift. If double
sewer cleanout is not detected during final plumbing test one will need to be installed for $500 at customers
expense.
With payment to be made as follows:

The price does not include any cosmetic repairs, plumbing, caulking, mortar repair, door adjustments, framing adjustments, unless specified provisions.
All material is guaranteed to be as specified, and the above work to be performed in accordance with the drawings and specifications submitted for
above work and completed in a substantial workmanlike manner. Integrity Foundation Repair is not responsible for any plumbing issues or cosmetic
damages as a result of the repairs preformed. Any alteration or deviation from the above specifications involving extra costs will be executed only upon
written order, and will become an extra charge over and above the contract. NOTE: this contract may be withdrawn by Integrity Foundation Repair if
not accepted within 14 days.
The above prices, specifications and conditions are satisfactory and are hereby accepted. You are authorized to do the work as specified. Payments
will be made as outlined above.

NOTES UNLESS OTHERWISE SPECIFIED
1) Exterior piling spacing should not exceed 6 feet center to center on two-
story homes and 7 feet center on one-story homes.
2) Pilings must be installed to refusal. Refusal is reached when the existing
foundation experiences uplift due to the driving force exerted on the piling.
3) Elevation readings shall be taken before and after the work is completed.
4) Elevations shall be adjusted for floor covering variations.
5) Pier depth log shall be maintained for all locations.
