� ACCIDENT RESPONSIBILITY RATING PLAN
ALL VEHICLE ACCIDENTS WILL BE CLASSIFIED. All traffic accidents invoiving an
employee of the City of Grapevine while operating a City owned vehicle, will be
classified according to the City of Grapevine Accident Responsibility Rating Plan.
CLASS I RATING. All accidents where a City vehicle is standing, parked at a curb or
in any position where it has a legal right to be.
NOTE: When the vehicle is parked in an unsafe manner or location to the
extent that it contributes to an accident, the incident should be
classed as Preventable and appropriate action taken.
ACTION: Non preventable, no action,
CLAS� II RATING. All accidents where the City vehicle is in motion but prevention
of the accident is beyond the control of the driver.
ACTION: Non preventable, no action.
CLASS III RATING. Accidents in which the City driver is partially to blame and failed
� to exercise preventative measures within the driver's control. Accidents that involved
a minor infraction of law or safety rules including the failure to report an accident.
NOTE: This does not preclude accidents that are reported late from being
assigned a higher classification if such action is warranted.
This would also be the minimum category for backing accidents (except those in
which mitigating factors are presentl. This category also includes accidents wherein
the City vehicle is a contributing factor because it was parked in an unsafe manner
or illegal location.
ACTION:
FIRST OFFENSE -- Appropriate administrative action. A letter of reprimand or
other action deemed necessary by the Department Head.
SECOND OFFENSE-- One (1 ) to three (3) days suspension without pay or other
appropriate disciplinary action as determined by the Department Head and/or
Director.
THIRD OFFENSE -- Suspension of driving for up to thirty (30) days as deemed

by the Director. Other appropriate action such as denial of next merit pay
• increase, derogatory comment on next employee evaluation or dismissal should
be considered by the Department Head/Director. .
` CLASS IV RATING. Accidents where the driver of the City vehicte is chiefly orwholly
to blame. This includes negligence, traffic' violations and/or misuse of the City
vehicle. Accidents that involve a major violation of laws or safety rules.
ACTION: �
FIRST OFFENSE -- One (1) to three (3) days suspension without pay or other
appropriate action as determined by the Director. Derogatory or adverse
comments should be made on employee's next evaluation form.
SECOND OFFENSE -- Three (3) to five (5) days suspension without pay or
other suitable disciplinary action as deemed appropriate by the Director. This
could include, but is not limited to, such action as suspension from driving for
up to thirty (30) days, denial of the next merit pay increase or dismissal.
THIRD OFFENSE -- Mandatory review of case by Director and Department
Head to determine appropriate disciplinary action. This could include five (5)
to fifteen (15) days suspension without pay, permanent revocation of driving
privileges, or dismissal.

CLASS V RATING. Flagrant violation of law or safety rules, excessive damage to
equipment or property or exceptional situations deemed by the Safety Review Board
to warrant special review including involuntary manslaughter or belligerent attitude.
ACTION:
FIRST OFFENSE -- Mandatory review of case by the Director and Department
Head to determine appropriate disciplinary action. Indefinite suspension of
driving privileges, suspension without pay up to fifteen (15) days, dismissal, or
other appropriate course of action.
SECOND OFFENSE-- Mandatory review of case by the Director and Department
Head to consider permanent denial of driving privileges.
THIRD OFFENSE -- Dismissal.
CLASS VI RATING. City driver has accumulated more than three (3) Class III
accidents in a twelve (12) month period; or has a combination of more than three (3)
Class III and Class IV accidents in a twelve (12) month period; or has five (5) Class

- - III or Class IV or a combination of Class III or Class IV accidents in an eighteen (18)
month period or has two (2) Class V accidents in a twelve (12) month period.
ACTION: Mandatory review of case by the Director and Department Head
to determine appropriate disciplinary action.
' DISCIPLINARY ACTION. A course of disciplinary action taken against an employee
involved in a traffic accident will be determined by the Department Head/Director.
The� penalties listed in the Accident Responsibility Rating Plan are recommended
guidelines and not to be considered all inclusive.
SAFETY REVIEW BOARD. The Safety Review Board is a committee composed of
management level personnel from various City Departments and is chaired by the Risk
Manager.
The Safety Review Board reviews all accidents involving City owned vehicles and the
ratirig classification given. .The Safety Review Board has the authority to recommend
changes to the City Manager regarding the current policy as well as the need for a
more consistent accident review by a particular department(s).
