This is a revision of Report 3 (03/14/19). Report 3 did not list the building or lot number associated with the work on site. This report is being issued to update the lot location where work was performed. The undersigned arrived on site, as requested, to observe the placement of 4000 psi concrete for Lot 5, Junction Box Top at Storm Drain Line 1, STA 0+00. Please see the attached sketch. The required test limits were determined to be a slump of 3-5 inches, air content of 3-6 %, batch to placement time of 90 minutes, and concrete temperature range of 55-95 degrees Fahrenheit. Concrete observed on this date appeared to be placed in compliance with project specifications with regard to temperature, slump, air content, and batch to placement time. One set of five and one individual laboratory cured concrete cylinders were cast for compressive strength testing in our laboratory. Prior to the placement of concrete, the reinforcing steel was observed and appeared to be placed in compliance with the project drawings and specifications.
