The undersigned arrived on site, as requested, to observe:
1. The compaction of soils for 12" Water Line, South of Buildings 1 and 2 from STA 1+00 to
3+00. Please see the attached sketch.
Utilizing the nuclear test method to check the compaction of soils; test results indicated that the
compacted material, at the areas and elevations tested, met or exceeded the project
requirements of 95% of the maximum dry density with a moisture content above optimum, as
obtained in our laboratory using the Standard Proctor Method (ASTM D-698).
Locations and elevations of all tests are based on stakeout provided by others. We cannot be
responsible for structures located off of the observed engineered pad, misaligned utilities or
stakeout errors causing uncontrolled fill to be placed in structural areas.
2. The placement of 4000 psi concrete for Lot 2R Storm Drain Line 6 Y-Inlets at STA 2+67,
STA 1+78, STA 0+67, Storm Drain Line 3 Grate Inlet, and Storm Drain Line 2 Curb Inlet.
Please see the attached sketches.
The required test limits were determined to be a slump of 3-5 inches, air content of 3-6 %,
batch to placement time of 90 minutes, and concrete temperature range of 55-90 degrees
Fahrenheit.
Concrete observed on this date appeared to be placed in compliance with project specifications
with regard to temperature, slump, and air content. THE BATCH TO FINAL PLACEMENT
TIME EXCEEDED THE 90 MINUTE DESIGN PARAMETERS.

One set of five laboratory cured concrete cylinders were cast for compressive strength testing in
our laboratory.
Prior to the placement of concrete, the reinforcing steel was observed and appeared to be placed
in compliance with the project drawings and specifications.
