Dear Applicant:
On July 26. 2018 a building application was submitted to this office for review for
the above referenced address. Our records indicate that your permit request
expired December 26, 2018. On February 26, 2019 you requested an extension.
That extension request started from the expiration date of December 26, 2018.
Your one-time extension expired June 26, 2019. If you wish to proceed with this
project new applications and plans must be submitted.
Performing work without a permit is a violation of city ordinance, regardless of
whether or not an application has been made, and could result in the revocation
of your contractor's registration for a period of not less than one year, and up to a
$2,000.00 fine. Additionally, failure to obtain your permit within 180 days of
application has resulted in the expiration of the permit.

Good Morning,
I just spoke with Seth,from Asian Chao regarding the remodel for Maki of Japan. Per our conversation,the remodel is
only cosmetic,and no kitchen equipment is being removed or added.Therefore,the Tarrant County Health Department
plan review is not needed. Maki of Japan has permission to proceed in its remodel. If any changes are made for the
remodel, please keep the health department updated.
