THIS IS TO CERTIFY THAT ON THIS DATE, I MADE AN ON-SITE INSPECTION OF
THE FOUNDATION AND DRAINAGE PERFORMANCE FOR THE ADDRESSED
PROPERTY. THE SUBJECT IS A TWO STORY, SINGLE FAMILY, BRICK AND STONE
VENEER DWELLING, SUPPORTED ON A SLAB-ON-GRADE FOUNDATION. FOR
ORIENTATION PURPOSES, IT IS ASSUMED THAT THE PROPERTY FACES WEST,
THIS INSPECTION MEETS THE REQUIREMENTS OF A "LEVEL B" EXAMINATION AS
DEFINED BY THE TEXAS BOARD OF PROFESSIONAL ENGINEERS.
SUMMARY OF OBSERVATIONS
Foundation
- This foundation is a reinforced concrete slab and grade beam type, constructed on native clay
soils.
Exterior
- There are several hairline tension cracks in the brick wall, as indicated on the attached
sketch.
- Grade at the front slopes toward the resident.
Interior
- Cracks on the slab of the garage.
- Cracks and separation of the Kitchen floor tiles.
- Crown molding separation on the ceiling of the Living Room.
- Cracks on floor tile of the Living Room.
- Separation between the Fire Place stone and the wall in the Living Room.
- Crack on the East wall of the Private Room of the Master Bedroom.
- Crack on the West wall of the Master Bedroom Bath.
- Crack at the corner of the Master Bedroom closet window.
- The Master Bedroom closet door is not aligned with its frame. CONTRACTOR SHALL CALL FOR
- Crack on the South wall of the Dining Room. INSPECTIONS:

- Separation at the Southeast corner of the 2nd garage.
- Crack on the ceiling of the 2nd garage.
- The interior of the house was surveyed using an electronic compu level. There is a
differential elevation in the range of 4.2"between the Northwest and Southeast corners of the
slab.
CONCLUSION
- This foundation experiences a combination of settlement and heave. Settlement along the
North and East sides and around the Northwest comer of the residence is due to shrinkage of
the expansive soils supporting the foundation in these areas. It requires repair at the present
time. The cause of the upheaval around the South section of the residence is unknown at the
present time. The heave can be caused by either a plumbing leak, poor surface drainage or
natural seepage problem.
RECOMMENDATION
- Install reinforced concrete piers under the perimeter beam, and under the interior slabs in the
locations shown on the attached sketch. These piers should be a minimum of 12" in diameter,
extending in depth to a minimum of 12 ft. or to a stable layer of soil or to bedrock, whichever
occurs first. The concrete should be 3000 psi strength and the reinforcing steel should be
Grade forty, (4)#4 rebar tied with#3 rings at 18" centers. After the piers have cured for at
least 7 days,mechanically raise the foundation to its original position or as practical. It
should be understood that damage to the plumbing system and cracking of rigid material such
as concrete,masonry and dry wall surface can result from leveling of the perimeter.
- Prior to raising the foundation, the contractor should test the water and sewer lines for leaks,
and again after raising,all of the lines should be checked for any possible damage or leak.
- Conduct two borehole tests at the East side of the residence in the locations shown on the
attached sketch. The borehole should be 4" in Dia. And extend to a depth of 12'. A 4"Dia.
Perforated pipe perforated P.V.0 pipe should be installed and the holes monitored for 6
months. The water table fluctuation should be monitored at least 6 months. A French drain
can be designed based on this borehole test if it is required.
- Follow the attached maintenance program at all times in order to maintain the moisture
content of the soil supporting the foundation to an optimum level.
NOTE:
- IN THE EVENT THAT OTHER INFORMATION BECOMES AVAILABLE THAT
COULD AFFECT THE CONCLUSION REACHED IN THIS INVESTIGATION, THIS
OFFICE RESERVES THE RIGHT TO REVIEW AND CHANGE SOME OR ALL OF
THE OPINONS REPRESENTED HEREIN, IF IT IS REQUIRED.
This inspection consists of visual observations only and is based on engineering judgment, made
solely to determine the foundation and drainage problem(s) of the described property at the
present time. This report does not predict or warrant the future performance of the inspected
foundation.

STRUCTURAL INSPECTIONS
The portion of the foundation which is inspected is readily visible without moving or removal of
any items that may cause a visual obstruction. In other words, this report is based solely on
observation of the apparent behavior of the structure.
Opinions expressed in this report are based on an engineering evaluation considering past
performance of the structure. It is assumed that the foundation was constructed based on the
requirements of building codes and was inspected by local building officials during construction.
No warranty is either expressed or implied as to the future performance of this structure.
Liability is strictly limited to the fee charged for this report.
In order to certify the repair works that are performed based on our specifications, the works
should be inspected after the pier holes are ready and rebar is in place, and again after raising and
before backfilling of the pier holes (this is to check that proper leveling of the foundation has
been done). The new elevations will be our reference for monitoring the future movement, if any,
of the foundation. These inspections require advance scheduling; therefore the engineer should
be informed at least 48 hours prior to the commencement of repair work. Additional engineering
fees will be charged for this service,which is not included in the fee for this report. (Each quality
control inspection fee is $200.)
This inspection is not intended to cover structural features above foundation level. This report
merely covers the status of the foundation at the time of this inspection.
Should you require a more in-depth study such as soil test, stress analysis and redesign, you need
only to advise our office. A reasonable fee will be quoted based on normal hourly charges for
engineering and consulting services.
